Nurse Case Manager II - CFHP

Position Summary:
Community First Health Plans is seeking a dynamic individual seeking employment in a hospital-affiliated health plan. Primarily, Monday through Friday from 8:30 a.m. to 5:00 p.m. work schedule. This Professional RN will assist Community First Health Plan (CFHP) members regain optimum health or improved functional capacity by ensuring that members have access to all the health care services they need in the most efficient and effective manner possible.

Qualifications:

• BSN preferred
• Four years acute care experience or managed care experience is required
• Current RN Texas License
• Certification from an appropriate professional agency such as CCM is required
