Gastroenterology RN Care Coordinator in Auburn, New York at Auburn Community Hospital

Position Summary:

The Care Coordinator provides individualized assistance, support, and comprehensive care coordination for patients with complex hepatology conditions. This role serves as a primary point of contact for patients and their families/caregivers, helping to eliminate barriers to timely care, facilitate care coordination across the health system, and enhance patient and provider satisfaction. The Nurse Navigator collaborates closely with a multidisciplinary care team to support high-risk patients and improve health outcomes and quality of life.

Responsibilities and Duties:

• Provide nurse case management for patients with complex hepatology conditions, including cirrhosis, autoimmune hepatitis, liver tumors, and other advanced liver diseases.
• Serve as the primary point of contact for patients and families, facilitating communication with providers and care teams.
• Develop, implement, and manage individualized care plans based on patient and provider goals.
• Coordinate appointments and care across multiple departments, including Transplant, Interventional Radiology, Oncology, Nutrition, Infusion, and other services.
• Manage a panel of high-risk hepatology patients, ensuring continuity of care and timely follow-up.
• Assist in clinic by providing patient education, support, and care coordination.
• Perform telephone triage and manage in-basket communications within the electronic medical record.
• Facilitate patient flow through the healthcare system and address barriers to care access.
• Collaborate with multidisciplinary teams to promote optimal care delivery and patient outcomes.
• Maintain accurate and timely documentation within the electronic medical record.
• Support patient engagement and satisfaction initiatives.

Qualifications, Education and Experience Requirements:

• Bachelor’s degree in Nursing (BSN) required.
• Current registered nurse (RN) licensure required.
• Minimum of 2–3 years of gastrointestinal (GI) nursing experience preferred.
• Experience caring for patients with complex or chronic medical conditions.
• Strong organizational, communication, and care coordination skills.
• Proficiency with electronic medical records.
Ability to work collaboratively in a multidisciplinary team environment.

Compensation Range:

$76,398.00 – $99,320.00 annually

Compensation will be determined based on experience.
