Job Description

Additional $1.00 per hour for working evening and / or night shift.

This is a professional nursing position involving responsibility for coordinating individual patient care and to provide ongoing healthcare services for the medically fragile with long-term disabilities, medical conditions, injuries, and illnesses, and for performing nursing services requiring substantial specialized judgement and skills within the Wayne County Nursing Home. Work performed under the general direction of a supervisory level nursing position and in accordance with a prescribed nursing care plan. Supervision exercised over the work of the Licensed Practical Nurses and other related auxiliary nursing personnel. Does related duties as required.
TYPICAL WORK ACTIVITIES:
  • Administering medications to residents
  • Monitoring Blood sugars and administering insulin to residents as directed
  • Helping with or creating comprehensive care plans for each resident with interdisciplinary team
  • Maintaining accurate resident charts and records, not limited to: MDS, Incident and Accident Reports, Evaluations
  • Assessing acute and/or critically ill residents and notifying the Provider as required
  • Assisting medical providers during resident examinations/rounds
  • Providing wound care, Applying dressings and bandages to wounds and monitoring progress
  • Updating and Counseling families and residents on condition and plan of care
  • Checking/monitoring vital signs, monitoring trends and updating Providers as necessary
  • Obtaining consents, Giving injections/maintaining vaccination status up to date
  • Supervising/leadership over Licensed Practical Nurses and other related auxiliary personnel
  • Helping with ADL’s, range of motion exercises
  • Setting up and administering IV therapy
  • Preparing residents for transfers (Home, hospital, another LTC facility, etc.)
  • Ordering and following up on lab work, x-rays and other diagnostic testing and addressing with Providers as required
  • Collaborating with outside medical specialists and/or outside agencies and the IDT/Providers (that team may include doctors, nurses, physical therapists, and other healthcare staff);
  • As a long-term care nurse, you will provide the day-to-day support necessary for your residents to stay comfortable and be as healthy as possible.

Good knowledge of nursing principles and techniques and their relation to medical and surgical practices and skill in their application; working knowledge of material medical, hospital dietetics, sanitation, and personal hygiene; skill in the application of nursing techniques and practices; ability to understand and follow technical oral and written instructions; ability to keep records and make reports; ability to get along well with patients; physicians and others; ability to plan and supervise the work of sub professional and non-technical personnel; good observation; mental alertness, firmness; initiative; cheerfulness; patience; emotional stability; sympathetic attitude toward the sick; good moral character; physical condition commensurate with the demands of the position.
MINIMUM QUALIFICATIONS: Graduation from a school of nursing approved by State Education Department from a course approved by such department qualifying for Registered Professional Nurse and Eligibility for a license issued by the State of NY to practice as a Registered Professional Nurse. Possession of the license at the time of appointment
SPECIAL REQUIREMENTS:
  • License must be maintained in good standing throughout employment.
  • Maintenance of a current CPR/BLS certification (American Heart Association or American Red Cross) is a mandatory condition of continued employment, per CMS (§483.35) and NYS (§415.13) regulations.
