We’re seeking a compassionate and experienced Advanced Practice Provider on a full-time or part-time basis in our Free Health Care Center in Downtown Pittsburgh.

The Advanced Practice Provider (APP) serves as a key clinical team member within the Catholic Charities Free Health Care Center (CCFHCC), providing high-quality, compassionate, and patient-centered care. The APP will deliver episodic and chronic disease management services, health screenings, patient education, care coordination, and preventive services within a collaborative, team-based environment. This position supports CCFHCC’s mission of improving access to care while fostering dignity, respect, and continuity of care for all patients.

Advanced Practice Provider Key Performance Objectives Include:

  • Provide direct patient care within the scope of licensure and CCFHCC clinical protocols.
  • Conduct patient assessments, obtain medical histories, perform physical examinations, and develop treatment plans.
  • Diagnose and manage acute and chronic medical conditions.
  • Order, interpret, and follow up on appropriate laboratory and diagnostic tests.
  • Prescribe medications and treatments in accordance with Pennsylvania law and clinic policies.
  • Provide preventive care services, patient counseling, and health education.
  • Coordinate referrals to specialty care, community resources, behavioral health services, and social support services as needed.
  • Document patient encounters accurately and timely within the electronic medical record (EMR).
  • Participate in quality improvement, risk management, and compliance activities.
  • Support integrated care initiatives, including collaboration with dental, behavioral health, and community partners.
  • Assist with development and implementation of clinical workflows and operational improvements.
  • Participate in team meetings, case discussions, and interdisciplinary care planning.
  • Support volunteer providers, students, and trainees as appropriate.
  • Comply with all CCFHCC policies and procedures, including HIPAA, FTCA requirements, infection prevention standards, and patient confidentiality expectations.
Requirements:

Minimum Qualifications:

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the Commonwealth of Pennsylvania.
  • Current DEA registration (if applicable to prescribing responsibilities).
  • Board certification in appropriate specialty area.
  • Current BLS certification required.
  • A minimum of two years of clinical experience preferred; experience in community health, primary care, free clinics, or underserved settings strongly preferred.
  • The ability to work collaboratively within a multidisciplinary team environment.
  • Strong interpersonal, organizational, and communication skills.
  • Commitment to serving vulnerable and underserved populations with compassion and cultural humility.
  • Act 33/34 and FBI clearances.

Preferred Experience

  • Experience working in nonprofit, community-based, or safety-net healthcare settings.
  • Familiarity with integrated care and social determinants of health.
  • Experience with EPIC or other electronic medical record systems.
  • Spanish or multilingual communication skills are a plus.

Physical Requirements

This position requires the ability to sit, stand, walk, bend, and perform routine clinical duties for extended periods. Reasonable accommodation may be made to enable individuals with disabilities to perform essential job functions.

Work Environment

The APP will work within a community-based clinical setting serving diverse patient populations. Evening clinic hours may occasionally be required based on operational needs.
