Position Description:
The Population Health Care Management Supervisor (RN) provides clinical and operational leadership for the organization’s longitudinal care management programs. This position is responsible for overseeing chronic disease management, care coordination, behavioral health integration, and future care management initiatives that support high-risk patient populations across the continuum of care.
The Supervisor leads the development and optimization of integrated care management workflows, ensuring patients receive coordinated, relationship-based care that improves outcomes, reduces avoidable utilization, and supports long-term disease self-management. This role partners closely with providers, leadership, and stakeholders to strengthen Population Health infrastructure while ensuring regulatory compliance, operational excellence, and continuous program improvement.
Responsibilities:
- Provide day-to-day leadership and supervision for assigned Population Health staff.
- Provide operational oversight of Chronic Care Management (CCM), Behavioral Health Integration (BHI), and other longitudinal care management programs that are based in comprehensive and holistic care.
- Lead development and optimization of workflows, documentation standards, and clinical protocols.
- Collaborate with leadership to develop and evolve integrated referral pathways between Population Health programs.
- Monitor program performance and implement workflow improvements that enhance efficiency, patient engagement, quality outcomes, and financial sustainability.
- Provide clinical consultation and guidance for complex patient situations and care coordination activities.
- Develop collaborative relationships with providers, practice leadership, internal departments, and community partners.
- Promote utilization of Population Health Services through provider engagement and education.
- Monitor quality, productivity, documentation, and operational performance for assigned programs.
Qualifications:
- Required
- Current New York State Registered Nurse (RN) license in good standing.
- Bachelor of Science in Nursing (BSN) required.
- Active Basic Life Support certification.
- Minimum five (5) years of progressive clinical nursing experience.
- One (1) to three (3) years of supervisory or leadership experience in a healthcare setting.
- Experience in care coordination, ambulatory care, chronic disease management, case management, or Population Health.
- Strong leadership, communication, organizational, and problem-solving skills.
- Proficiency with electronic health records and Microsoft Office applications.
- Preferred
- Master's degree in Nursing, Healthcare Administration, Public Health, or related field.
- Certification in Case Management, Care Coordination, Ambulatory Care Nursing, or Population Health.
- Experience with Medicare care management programs and practice transformation initiatives.
- Experience leading multidisciplinary clinical teams.
Pay Range: $62,354 – $83,200.00
