Department: Case Management/Community Engagement

Reports to: Case Management/Community Engagement Coordinator

FLSA: Non-Exempt, full time, 40 hours/week

Level: 5

Date: August 2026

Summary: The ACCESS York Case Manager provides trauma-informed, survivor-centered case management and advocacy to individuals and families impacted by domestic violence. The position supports survivor safety, stability, self-sufficiency, and access to community resources while maintaining confidentiality and survivor choice.

ESSENTIAL DUTIES

  • Conduct intake, needs assessments, safety planning, and individualized service planning.
  • Provide ongoing case management, crisis support, advocacy, referrals, and follow-up services.
  • Assist survivors with housing, employment, education, benefits, transportation, childcare, legal resources, and other identified needs.
  • Coordinate referrals and warm handoffs to YWCA York programs and community partners.
  • Support eligible participants with housing and supportive-service programs, including the Next Door Program when applicable.
  • Maintain timely, accurate, objective, and confidential case documentation and required program data.
  • Participate in case conferencing, supervision, staff meetings, training, and program improvement activities.
  • Maintain knowledge of community resources and perform other related duties consistent with the position and YWCA York mission.
  • Participate in external community activities, outreach initiatives, and events that promote communication, awareness, and education regarding domestic violence, available services, and community resources.

CONFIDENTIALITY AND SURVIVOR-CENTERED PRACTICE

  • Protect survivor information and follow applicable confidentiality, VAWA, mandated reporting, and organizational requirements.
  • Provide services that promote safety, dignity, cultural responsiveness, empowerment, voice, and choice.
  • Respect each survivor's right to make decisions regarding relationships, housing, legal action, and participation in services.

QUALIFICATIONS

  • College degree preferred.
  • Experience in case management, advocacy, crisis response, housing services, victim services, or related human services preferred.
  • Knowledge of domestic violence, trauma-informed care, housing instability, and community resources preferred.
  • Strong communication, organization, documentation, problem-solving, and time-management skills.
  • Ability to maintain confidentiality, professional boundaries, and sound judgment while working with individuals experiencing crisis.
  • Ability to work independently and collaboratively and to use standard electronic documentation and office technology.

The Case Manager is expected to demonstrate professionalism, reliability, flexibility, and respect; follow YWCA York policies and safety practices; and support the organization's mission of eliminating racism and empowering women. Some local travel and schedule flexibility may be required.

PHYSICAL REQUIREMENTS

  • Ability to lift, push, and/or pull up to 25 pounds.
  • May require driving in inclement weather.
  • Frequent walking, sitting, standing and typing.
