Job Description
JOB SUMMARY:
Provide routine individualized healthcare to the elderly, convalescents, or person with disabilities at the patient’s home or in a care facility. Monitor or report changes in health status. May also provide personal care such as bathing, dressing, and grooming of patient.
ESSENTIAL DUTIES AND RESPONSIBILITIES: All duties of the Home Health Aide are under the direction of a Registered Nurse or Therapist as follows:

§ Assist with personal care, which shall include assisting the client with personal hygiene, bathing, dressing, grooming, ambulation, eating, shaving and physical transfer.
§ Assist with the change of a colostomy bag and reinforcement of dressing, if instructed in procedure by an RN.
§ Assist with the use of an aid for daily living, i.e., walker, cane, wheelchair.
§ Assist with prescribed range of motion exercises which have been taught by a health care professional of the agency.
§ Take vital signs (TPR) as directed.
§ Perform catheter care as directed.
§ Measure intake and output of fluid.
§ Keep records of personal health care and activities.
§ Submit patient’s notes & paperwork on a weekly basis
Supervision of self administered medication in the home or healthcare by:
§ Remind patient of the time to take the medication as prescribed.
§ Prepare necessary items such as juice, water or spoons to assist client in the self administration of the medication.
§ Provide homemaking and light housekeeping of the patient’s care area as directed.
§ Prepare meal services as directed.
