Job Description

Position Summary: The Transformation of Rural Community Health Care (ToRCH Care) Anchor Hub 27 Coordinator supports daily hub operations, partner communication, referral workflows, documentation, reporting, and care coordination activities. This non-clinical role serves as a central contact for healthcare, behavioral health, social care, and community partners to improve collaboration and address health-related social needs across the region.

Essential Duties and Responsibilities:
  • Coordinate hub operations, meetings, partner communication, follow-up tasks, and implementation activities.
  • Serve as a liaison among hub leadership, providers, community organizations, referral partners, clients, and stakeholders.
  • Support intake, referrals, resource navigation, outreach, and follow-up related to health-related social needs.
  • Maintain documentation, track program activity, monitor deadlines, and assist with reports, agendas, minutes, and presentations.
  • Identify workflow barriers, service gaps, documentation issues, or partner concerns and escalate them appropriately.
Qualifications:
  • Associate degree or equivalent education and experience in healthcare, social services, public health, care coordination, community health, patient navigation, case management, or a related field.
  • Knowledge of care coordination, community resources, social determinants of health, Medicaid or high-need populations, and referral workflows.
  • Strong organization, communication, analytical, and follow-up skills with the ability to manage multiple priorities.
  • Proficient with Microsoft Office, spreadsheets, dashboards, referral systems, data entry platforms, and confidential information.
Preferred Skills and Abilities:
  • Bachelor’s degree in public health, healthcare administration, social work, health informatics, or a related field preferred.
  • Familiarity with community resources, Medicaid populations, healthcare systems, and regional service networks.
  • Strong written, verbal, and interpersonal skills with the ability to engage diverse clients, partners, providers, and stakeholders.
  • Ability to work independently and collaboratively while identifying gaps, barriers, and follow-up needs.
Physical and Work Environment Requirements:
  • Work is typically performed in an office, clinic, community, or hybrid coordination setting.
  • Ability to use standard office equipment, work on a computer, and communicate by phone, video, and in person for extended periods.
  • Local travel may be required for partner meetings, outreach, training, or community activities.
  • Must work independently while supporting communication, coordination, data use, and continuous improvement across the hub.
