Employment Type: Full Time

Shift/Schedule: Day Shift

About the Role

The Stroke Coordinator will be responsible for the management of services and systems necessary for a multidisciplinary approach to providing care to Stroke patients. This individual establishes relationships with all related physicians, professionals or other health related groups within the community and collaborates with the neurology and cardiovascular physicians, Nursing Director, unit staff and other team members to ensure all metrics are met to establish and maintain Saratoga Hospital as a Center of Excellence for Stroke.

The position requires knowledge and skill of professional nursing theory and practice, stroke care and systems, principles of teaching and learning, research, and quality assurance and management.

What You’ll Do

  • Collaborate with the Neurology Medical Director, and Cardiovascular & Pulmonary Services Nursing Director to achieve this goal.
  • Report findings to Medical and Nursing Directors on a usual cadence i.e., Monthly or every other month, to discuss areas of achievement along with areas for improvement.
  • Attend Stroke codes to assess in real time, the current process, (min. 1 per week) and provide feedback and coordinate follow up education, if necessary, establish meetings with the EMS community on a routine basis (quarterly) to provide feedback on performance and areas for improvement in both Stroke cases, attend monthly Cath Conference
  • Establish and maintain a standing meeting for M&M review with Quality and leadership to review cases where there was morbidity or mortality.
  • Ensure data being collected is accurate and submitted based on the deadlines provided by each regulatory agency, also provide regular updates to Director of Quality
  • Real time evaluation of the process and interventions to improve outcomes, in services, Debriefings like the reviews performed after every code.

What You Bring:

  • NYS RN license required, Bachelors required, Masters preferred.
  • Minimum 5 years working in an acute care setting, emergency nursing, or critical care required, experience with care of the stroke patient preferred.
