Job Description

If so, we have an exciting opportunity for you to join our team as a Program Supervisor - Care Coordination. This position offers a hybrid work environment with responsibilities that include office, community, and remote work.

What You Will Be Doing

As the Program Supervisor - Care Coordination, you will lead and supervise Care Coordination services, provide clinical supervision as needed, and oversee key activities such as hospital continued stay reviews, discharge planning, and coordinated transitions of care.

You will work closely with health plans, the PIHP, providers, Primary Clinicians, Access staff, Designated Collaborating Organizations, and other partners to support joint care planning, compliance, and integrated care delivery.

This role also serves as a resource for complex cases, providing consultation, resolving service or communication concerns, monitoring documentation and service quality, and supporting quality improvement efforts.

Additional responsibilities include leading the LOCUS Integrity Team, participating in MI FAST audits and Plans of Correction, facilitating partner meetings, supporting policy and procedure development, assisting with complaint resolution, and contributing to committees and organizational initiatives.

What We Require
A Master’s degree licensed professional with a minimum of three years’ experience in behavioral health care services. Experience with care coordination, collaboration with health plans, and utilization management preferred. Experience in supervision with demonstrated leadership skills and an ability to train/mentor other team members is preferred. You must have one of the following Michigan Licensures: Licensed Master Social Work (LMSW) or Licensed Professional Counselor (LPC). Limited licenses will be considered. A valid Michigan Driver’s License is required.

A strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, serious emotional disturbance, and substance use disorders is needed. You will understand how to implement a strong recovery philosophy and be able to collaborate and develop teams.

Physical Requirements/Working Conditions:
Physical Efforts
– Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot.

Working Conditions – Office environment with noise from computers, copy machine, and telephones. Use of computer for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of computer screen. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
