Livingston Memorial is looking for a Full Time, 40 hour a week, Community Health Worker to support our Clinical Team.

Position Summary:

The Community Health Worker (CHW) is responsible for assisting patients and their families to navigate and access Medi-Cal, community services, other resources, and adopt healthy behaviors. The CHW supports the Community Based Palliative Care Team, Home Health and Hospice Social Workers through an integrated approach to care management and community outreach.

What You'll Do

  • Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families.
  • Responsible for establishing trusting relationships with patients and their families while providing support as outlined in the plan of care.
  • Follow up with patients via home visits and visits to other settings where patients can be found.
  • Help patients set personal health related goals and attend appointments.
  • Provide referrals for services to community agencies as appropriate.
  • Help patients connect with transportation and other resources necessary to meet healthcare appointments.
  • Work closely with medical providers to help ensure that patients have comprehensive and coordinated care plans.
  • Work collaboratively with other clinical personnel.
  • Act as a patient advocate and liaison between patient/family and community service agencies.
  • Record patient care management information in the EMR within 24 hours after direct patient contact.
  • Attend staff meetings, trainings, and other meetings, as requested.
  • Maintain HIPPA compliance at all times.
  • Performs other duties as assigned.
Requirements:

  • High school diploma or equivalent
  • Bilingual in Spanish
  • Drivers License
  • Auto Insurance
  • CHW Certificate, preferred
  • Culturally aligns with the population being served.
  • Abilities to work independently and be self-directed and flexible.
  • Good organizational skills to handle multiple priorities while remaining professional and calm.
  • Knowledge of computer, proficient with Microsoft Word and Excel.
  • Good verbal and written communication skills.
  • High level of attention to details.
  • Able to work independently, take ownership and responsibility of tasks.
  • Strong professional ethics, discretion and judgment.
  • Good organizational skills.
