Summary:The CPS nurse supports the NH CPS program, ensuring that all files are up to date with yearly physicals and other required documentation. The nurse ensures that all monthly quality assurance (QA) records are completed, all orders are on file as needed, and staff are trained as needed. You will have up to 32 hours of direct service and 8 hours of other duties and responsibilities. Each Direct Support Professional (DSP) works with a small group of program members each day, engaging them in community outings as well as a variety of classes led by colleagues, including art, music, reading & writing skills, yoga, and other interests & goals set by the individuals themselves every year.
Essential Duties and Responsibilities:- Always ensure the health and safety of members
- Timely completion of all required paperwork (i.e.: email correspondence, daily notes, mileage logs, timecards, progress notes and incident reports).
- Complete QAs on time
- Ensure all physicals are up to date and issue reminders as necessary.
- Complete monthly AED checks and other safety equipment checks regularly.
- Assist with writing protocols for members
- Administer the 1201 test
- Assist CPS program manager with yearly certification documentation and be available in office on day of certification to assist
- Be a positive role model with strong advocacy toward member rights and choice.
- Attend and participate in all required meetings (i.e. staff meetings, ISA meetings).
- Support individuals of all abilities including those with personal care needs and behavioral accommodations.
- Exposure to bodily fluids noting that Great Bay Services uses universal precautions.
- Excellent written and verbal communication skills
- Provide instructional support while assisting members with the implementation of their yearly individual service agreement while encouraging independence.
- Provide transportation for members in the community, often using your own personal vehicle (mileage in reimbursed).
- New Hampshire or Maine LPN required. RN Desired.
- Completion of TB Test
- Prior experience working with individuals with intellectual and/or developmental disabilities preferred
- First Aid and CPR certified; training will be provided if needed
- He-M 1201 Administration of Medications in Developmental Services Programs training will be provided to those candidates without it

- Must pass routine background checks including criminal, motor vehicle, DHHS, and others required to comply with Medicaid funding.
