Job Description
Southern Oregon Wellness is seeking a compassionate, patient-centered Family Nurse Practitioner (FNP) to join our direct primary care practice in Southern Oregon.

Southern Oregon Wellness is a direct primary care practice focused on providing accessible, relationship-based healthcare. Our model allows clinicians to spend more time with patients, emphasize prevention and wellness, and develop long-term relationships rather than focusing on high-volume visits.
Position
Family Nurse Practitioner (FNP)
Employment: Full-time or part-time
Location: Southern Oregon
Practice Model: Direct Primary Care
What You’ll Do
Provide comprehensive primary care to patients across the lifespan
Diagnose and manage acute and chronic medical conditions
Perform preventive health evaluations and wellness visits
Develop individualized treatment and health-maintenance plans
Provide patient education, counseling, and lifestyle guidance
Build lasting relationships with patients and families
Order and interpret appropriate laboratory and diagnostic studies
Coordinate specialty care when needed
Participate in improving the patient experience and practice processes
Practice with an emphasis on prevention, whole-person care, and evidence-based medicine
What We’re Looking For
Current Family Nurse Practitioner (FNP) certification
Active Oregon NP license and appropriate prescribing privileges
Graduate of an accredited nurse practitioner program
Current national board certification
Strong clinical assessment and decision-making skills
Excellent communication and interpersonal skills
A genuine interest in relationship-based, preventive healthcare
Comfortable working independently while collaborating with the broader care team
Commitment to providing compassionate, personalized care
New graduates with strong primary-care training and an interest in direct primary care are encouraged to apply.
