Key Responsibilities

• In-Home Patient Care: Deliver high-quality direct nursing care to patients across Montgomery, Clark, and Greene counties.
• Medication Management: Safely administer medications as prescribed by healthcare providers and monitor efficacy.
• Dementia & Cognitive Care: Implement specialized dementia protocols to support patients experiencing cognitive decline.
• Clinical Assessments & Interventions: Monitor vital signs and overall patient condition, applying clinical physiology knowledge to track changes in status.
• Wound & Urgent Care: Perform complex wound care and respond effectively to urgent clinical situations.
• Documentation & Coding: Maintain precise patient medical records utilizing ICD-10 coding standards.
• Care Coordination: Partner closely with physicians, RN case managers, and hospice aides to ensure seamless care plans.

Qualifications & Requirements

• Licensure: Valid LPN license in the State of Ohio (in good standing).
• Experience: Proven nursing experience in home health, hospice, or long-term care / nursing home settings.
• Clinical Skills: Strong foundation in medication administration, urgent care response, and clinical physiology assessment.
• Specialized Knowledge: Familiarity with dementia care best practices and working knowledge of medical documentation/ICD-10 coding.
• Travel: Valid driver’s license, reliable transportation, and active auto insurance to cover the service area (Montgomery, Miami, and Greene counties).
