Public Health Program Coordinator in California, Maryland at Wellness and Equity Alliance LLC

Wellness Equity Alliance (WEA) is a California-based, mission-driven healthcare organization dedicated to closing gaps in healthcare access for underserved and marginalized populations.

The Public Health Program Coordinator plays a critical role in supporting the implementation, quality, and operational integrity of WEA's integrated care programs, including Street Medicine, Enhanced Care Management (ECM), Housing Navigation, and Integrated Behavioral Health. The position also supports broader public health initiatives across the organization.

Programs supported may include primary care, infectious disease prevention and treatment, gender-affirming care, harm reduction, substance use disorder prevention and treatment, behavioral health services, housing navigation, and care coordination.

A key function of this position is helping ensure that documentation, workflows, and program operations support effective revenue cycle management and reimbursement capture. The Program Coordinator works collaboratively with billing, compliance, clinical, and operational teams to identify documentation gaps, reduce preventable claim denials, and support sustainable program funding.

This is a highly cross-functional role well suited for someone who is comfortable working across clinical, operational, compliance, and public health functions within a fast-paced and evolving environment.

Program Implementation & Quality

• Support day-to-day coordination of Street Medicine, ECM, Housing Navigation, and Integrated Behavioral Health programs across assigned regions and counties.
• Monitor program workflows for alignment with contractual, regulatory, and payer requirements, including Medi-Cal, CalAIM, and managed care plan requirements.
• Track and report program performance metrics, quality indicators, and outcomes.
• Identify operational or documentation gaps and assist with corrective action planning.
• Assist with the development, refinement, and implementation of standard operating procedures, workflows, and training materials.
• Support onboarding and training of new field staff, including community health workers, care managers, and clinicians, on documentation and workflow requirements.
• Conduct chart audits and documentation reviews to support quality, completeness, and compliance.

Housing Navigation & Care Integration

• Support coordination among Street Medicine, Behavioral Health, and Housing Navigation teams to facilitate warm handoffs and continuity of care.
• Assist with tracking housing navigation referrals, milestones, and outcomes.
• Help identify and resolve barriers affecting service integration across multidisciplinary teams.

Public Health Program Support

• Provide coordination support for additional public health initiatives as assigned, including infectious disease response, harm reduction campaigns, community health events, and vaccination initiatives.
• Assist with data collection, reporting, and grant or contract deliverables related to public health programming.
• Support community partnerships and stakeholder engagement as needed.

Administrative & Cross-Functional Support

• Prepare reports, dashboards, and presentations for internal leadership and external funders or payers.
• Maintain accurate records within EHR and other data systems used for program tracking and reporting.
• Participate in regular meetings with the Director of Street Medicine and Senior Public Health Project Manager to coordinate priorities and assignments.
• Support special projects and other program-related responsibilities as assigned.

• Bachelor's degree in Public Health, Health Administration, Social Work, Nursing, or a related field, or equivalent professional experience.
• At least two years of experience in healthcare program coordination, care coordination, public health, healthcare operations, or a related role.
• Experience working with or supporting programs serving underserved populations, individuals experiencing homelessness, harm reduction programs, behavioral health services, or similar populations.
• Basic understanding of healthcare billing and reimbursement concepts, or a strong willingness and ability to learn revenue cycle management fundamentals.
• Strong organizational skills and the ability to independently manage multiple workflows and competing priorities.
• Strong written and verbal communication skills.
• Ability to work effectively in a remote, fast-paced environment with limited daily supervision.
• Proficiency with Google Workspace, EHR systems, Excel, and/or other data tracking and project management platforms.

• Experience with California Medi-Cal, CalAIM, Enhanced Care Management, or managed care documentation requirements.
• Familiarity with street medicine, infectious disease programs such as HIV or HCV services, gender-affirming care, or substance use disorder treatment environments.
• Experience conducting chart audits, quality assurance reviews, documentation reviews, or compliance monitoring.
• Bilingual proficiency in Spanish or another language commonly spoken within the communities WEA serves is a plus.

• Fully remote position.
• Participation in occasional virtual meetings during Pacific Time business hours is required.
• Flexible part-time schedule averaging approximately 8–16 hours per week.
• Weekly hours may fluctuate depending on program needs, grant requirements, contract deadlines, and organizational priorities.
• Occasional regional travel or in-person meetings may be requested. Approved business-related travel will be compensated in accordance with applicable WEA policies.
