About This Position

Job Title: Nurse Practitioner – Inpatient Palliative Care
Location: Northwest Hospital, 5401 Old Court Rd, Randallstown, MD
Schedule: Full-Time | 4x10-Hour Shifts (8:00 AM – 6:00 PM) | Includes One Weekend per Month

About the Role:
Northwest Hospital is seeking a compassionate and experienced Nurse Practitioner to join our Inpatient Palliative Care team. This full-time position offers a rewarding opportunity to provide high-quality care and support to patients and families facing serious illness.

Key Responsibilities:

  • Conduct face-to-face patient encounters in inpatient and home hospice settings.

  • Perform health status data collection in a comprehensive, systematic manner.

  • Collaborate closely with the Medical Director and Associate Medical Director on patient visits and care plans.

  • Complete certification and recertification assessments for hospice patients.

  • Submit documentation within 48 hours of patient visit and communicate findings to leadership.

  • Conduct daily inpatient rounds and complete death certificates as required.

  • Adhere to Hospice Conditions of Participation and maintain compliance with regulatory standards.

  • Collaborate with Hospice and Palliative Care Directors and interdisciplinary team members to ensure operational excellence.

  • Provide physician backup including prescription writing (e.g., comfort care kits).

  • Deliver educational initiatives internally and externally, as needed.

  • Offer palliative care consults, recommend treatment adjustments, and coordinate care with primary care physicians.

Requirements:

  • Active Nurse Practitioner License (NP) from the Maryland Board of Nursing (MBON)

  • Basic Life Support (BLS) certification from the American Heart Association

  • Prior experience in hospice, palliative care, or related medical fields is highly desirable
