About This Position
Description:
Position Summary
The Admissions Director is responsible for overseeing the admission process for the long-term care facility to ensure timely, accurate, and appropriate resident admissions. This position serves as the primary point of contact for referrals, hospitals, families, and other community partners, while coordinating closely with the interdisciplinary team to support census growth, smooth transitions, and positive resident and family experiences. The Admissions Director also helps ensure all admissions are clinically appropriate, financially verified, and compliant with facility policies and regulatory requirements.
Essential Duties and Responsibilities
- Manage the entire admissions process from referral through admission and room placement.
- Review incoming referrals for appropriateness, payer source, level of care, and bed availability.
- Communicate with hospitals, case managers, discharge planners, physicians, families, and internal staff regarding admission status and admission criteria.
- Coordinate with nursing, social services, therapy, dietary, transportation, and other departments to ensure a safe and successful admission.
- Verify insurance coverage, payer eligibility, authorizations, and required documentation prior to admission when applicable.
- Maintain accurate census records, referral logs, admission documentation, and daily admission activity reports.
- Conduct facility tours for potential residents and families and provide information regarding services offered.
- Develop and maintain positive relationships with referral sources and community partners to support census development.
- Participate in marketing and outreach efforts to promote facility services and maintain strong referral pipelines.
- Ensure all admission paperwork is completed timely and accurately, including consents, financial forms, and required regulatory documents.
- Collaborate with the business office to ensure accurate financial intake and payer source verification.
- Communicate bed status and admission updates to facility leadership on an ongoing basis.
- Assist with readmissions, transfers, and coordination of specialized placements as needed.
- Monitor admission trends, referral outcomes, and barriers to admission, and provide recommendations for process improvement.
- Ensure resident rights, confidentiality, and HIPAA standards are maintained throughout the admissions process.
- Participate in meetings related to census, care coordination, quality improvement, and facility operations.
Knowledge, Skills, and Abilities
- Knowledge of long-term care admission practices and regulatory expectations.
- Ability to evaluate referrals and communicate admission decisions clearly and professionally.
- Strong problem-solving and follow-up skills.
- Ability to manage sensitive conversations with families and referral sources with compassion and professionalism.
- Strong attention to detail and documentation accuracy.
- Ability to support census goals while maintaining appropriate clinical and financial screening standards.
Qualifications
- Minimum of 2 years of experience in admissions, case management, marketing, social services, or healthcare business development preferred.
- Long-term care, skilled nursing, rehabilitation, or healthcare experience strongly preferred.
- Knowledge of Medicare, Medicaid, managed care, and private pay processes preferred.
- Strong communication, customer service, and relationship-building skills.
- Ability to work effectively with hospitals, families, residents, physicians, and interdisciplinary teams.
- Strong organizational skills and ability to manage multiple referrals and priorities in a fast-paced environment.
- Proficient in electronic medical records, email, spreadsheets, and basic office systems.
- Ability to maintain confidentiality and professionalism at all times.
