Palliative Care Registered Nurse (RN) - Full Time

Service Area: Lee County, FL (Fort Myers, Cape Coral, Bonita Springs & surrounding areas)

Preferred Care Home Health is seeking a compassionate, experienced Palliative Care RN to join our interdisciplinary team serving patients and families throughout Lee County. In this role, you'll act as Case Manager delivering comprehensive, patient-centered nursing care in the comfort of patients' homes and communities.

What You'll Do

• Conduct comprehensive initial and ongoing patient/family assessments
• Develop and update individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments as ordered
• Educate patients and families on advance care planning, disease management, and self-care
• Coordinate care with physicians, community resources, and team members
• Supervise home care aides and support ancillary staff
• Participate in on-call rotation and interdisciplinary team meetings

What We're Looking For

• Active Florida RN license
• Graduate of an NLN-accredited nursing program
• Minimum 2 years of nursing experience, including at least 1 year in home care, palliative care, or hospice
• Palliative care/hospice training or certification preferred (or eligibility for certification)
• Current CPR certification
• Valid driver's license, reliable insured vehicle
• Strong communication, critical thinking, and organizational skills
• Ability to work independently with minimal supervision
• Physical ability to walk, stand, bend, and assist with patient transfers
