Position Overview

Reporting to the Vice President of Home Care and Hospice, the Hospice Admission Coordinator is responsible for setting up optimal conditions for the admission process, as well as the delivery of patient care with a focus on appropriate interdepartmental support, collaboration and cadence of communication with all those who take part in the full life cycle of care delivery. He/she/they ensure collaborative and responsive relationships with referral sources, which include responding to all inquiries, coordinating the referral process, and assisting with the patient intake process for Hospice.

What You'll Do

  • Coordinates and collects information for patient referrals from account partners to ensure smooth transitions of care from facility to home or hospice house with Granite VNA services.
  • Actively collaborates with patients and families as well as Agency staff; provides education and resources on transitions of care, to include hospice services. Ensure that they are given an opportunity to ask questions and provide input.
  • Takes all the necessary steps to facilitate a smooth transition to hospice care, ensuring that all the patient's needs are met by the assigned clinical care team.
  • Ensures consents are sent to the responsible party and all documentation is received back and in order prior to admission.
  • Schedules all new SOC visits.
  • Manages and oversees the Granite VNA referral email group to facilitate timely admission.
  • Daily reconciliation of Concord Hospital dischargers and transfers; includes adding the appropriate liaison and event code to ensure the team stays informed on patient status with updated reports.
  • Demonstrates understanding of visit frequencies and certification periods in order to problem solve in conjunction with the clinical team to schedule accurately and effectively.
  • Serves as the backup to scheduling should visits need to be canceled or rescheduled to include communication with staff as appropriate, as well as patients.
  • Documents all changes to visit status and notes accordingly in the system of record.
  • Addresses inquiries from the community about hospice services.
  • Communicates clearly with professional staff regarding expected “call backs” and relay information to physicians and others to take accurate messages.
  • Works with Hospitals outside of Granite VNA’s coverage areas to coordinate patient referrals to ensure smooth transitions of care.
  • Maintains a comprehensive working knowledge of community resources and referral resources and assists referral sources in accessing those resources if indicated.

  • Understands and demonstrates compliance with agency policies and procedures
  • Complies with infection control and safety measures in accordance with agency policy and procedure
  • Completes all the agency’s policies regarding clients’ privacy, confidentiality, and HIPAA requirements
  • Adheres to the Agency’s standard of professional appearance and the Agency’s dress code policy
  • Is current with all agency employment requirements required for the position (TB, licensure, annual in-services, and CPR).
  • Performs job duties in a safe manner in order to prevent injury to yourself and others.

This is NOT a remote work position, but may be eligible for work from home as outlined in the Agency’s guidelines.

Expected Hours of Work

Monday–Friday: 8am – 5pm and may change based on Agency needs. Work assignments may include holidays.

Requirements:
  1. Bachelor’s degree in Nursing, Physical Therapy, Occupational Therapy, Social Work, or a health care-related field or equivalent combination of education and work experience.
  2. Previous experience in customer service, sales, or care coordination
  3. Ability to navigate sensitive and often complex discussions by providing information, insight, and an appropriate level of support for the need.
  4. Able to handle competing priorities and manage those that are time sensitive successfully.
  5. Able to work independently and apply critical thinking skills to complex situations, often involving escalated emotional states of patients and families
  6. Proficient in the use of MS Office
