Care Manager at Willis-Knighton Health System – Shreveport, Louisiana

The nurse Care Manager will coordinate and manage the care throughout the patient's acute inpatient episode of illness; ensure continuity of services and appropriate utilization and management of resources.

Demonstrate leadership abilities by exhibiting a proactive approach to the care management process. The case management processes and key elements involved include planning, facilitating, advocating, educating, monitoring, evaluating and measuring the various outcomes for each identified patient population. Demonstrate a commitment to group goals and teamwork. Assumes responsibility and accountability for your own duties and performance.

QUALIFICATIONS:

Valid Louisiana nursing license (RN)

Prefer a minimum of 2 years clinical experience in an acute care setting, Preferably with management experience. Utilization and/or Case Management experience is desirable.
