Position Summary

We are looking for a detail-oriented and highly organized Referral Specialist to join our team at Exechealth. In this role, you will be responsible for managing and coordinating patient referrals to ensure smooth and efficient care transitions between healthcare providers. Your work will be essential in facilitating timely and accurate referrals, improving patient outcomes, and supporting our healthcare partners.

What you will be doing

- Manage Referral Process: Oversee the referral process from start to finish, ensuring all necessary documentation is complete, accurate, and submitted in a timely manner.

- Coordinate with Healthcare Providers: Communicate effectively with physicians, specialists, and healthcare teams to schedule and follow up on patient referrals.

- Track and Document Referrals: Maintain a comprehensive referral tracking system to monitor the progress and outcome of each referral. Document all referral activities and outcomes in the patient’s record.

- Patient Support: Provide patients with information on the referral process, answer questions, and assist with scheduling appointments as necessary.

- Ensure Compliance: Ensure all referral activities comply with healthcare regulations, patient privacy laws (e.g., HIPAA), and company policies.

- Collaborate with Internal Teams: Work closely with other departments such as clinical, billing, and patient services to ensure a seamless experience for patients.

- Resolve Issues: Address any issues or delays in the referral process and work to resolve them promptly to minimize impact on patient care.

You have

• Previous MOA experience is preferred.

• At least 1 year of experience in referrals in the family health office environment.

• Strong organizational skills and attention to detail.

• Excellent communication and patient education skills

• Knowledge with OSCAR EMR required.
