Job Summary:
The Stroke Advanced Practice Provider (APP) for the NJ designated Primary Plus Stroke Center will be a certified Advanced Practice Nurse (APN)/Nurse Practitioner
The APN will provide the following but not limited to: acute stroke patient care management including diagnosis, assessments and documentation, consultations for general and stroke neurology patients, daily patient rounds, assists with inpatient staff development/education, research trial patient enrollment and management, billing for all payers, collects and maintains quality metrics via national database, and community outreach
Job Responsibilities:
- APNs will perform according to the Nurse Practice Act of New Jersey and State Board of Nursing; maintain a Joint Protocol with a collaborating Physician for prescriptive authority
- APN will perform under the supervision of the attending Neurologist and in accordance with the objectives, policies and procedures of the hospital
- Serves as nursing leader, consultant, researcher, educator and direct care provider
- As Consultant, the APN facilitates problem solving with individuals, families, group agencies, communities and/or interdisciplinary colleagues (within and outside of the clinical setting)
- As an Educator, the APN promotes learning efforts of clients, staff, and students within the clinical setting, area schools and institutions of higher learning
- The APN as Researcher promotes scientific inquiry in clinical practice and uses research process and findings to improve nursing practice
- The APN as Practitioner, models excellence in practice through direct care to selected patients or groups
- Examines and evaluates patient s health status by performing an advanced physical exam and obtaining a comprehensive health history
- Maintains clear and accurate documentation according to hospital policy
- Initiates and performs selected therapeutic and diagnostic procedures
- Prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area
- Anticipates needs of the patient and evaluates the plan of care for the patient, and makes modifications as indicated
- Reviews discharge needs of the patient and family by participating in discharge planning and transitions of care
- Participates in team meetings and conferences when available, to enhance an interdisciplinary approach to health care delivery
- Performs a patient record review with the collaborating physicians
- Participates and supports in identifying and educating learning needs of staff
- Supports transitions of care clinic for outpatient follow up of Neuro patients discharged from hospital and out in the community
Requirements:
- Masters prepared in Nursing from an accredited program
- Advanced Practice Nurse License via the New Jersey State Board of Nursing
- Maintain National Certification AANP or ANCC
- Licensed Registered Nurse in the state of New Jersey
- Successful completion of all required orientation and training programs (see Job Function Category)
- 8-hour Education Requirement in the area of cerebrovascular disease annually
- Intravenous therapy
- Ideal candidates will have minimum of one year of experience as an Advanced Practice Provider in an acute care setting; experience with acute stroke and/or neurological patients preferred
- BLS/ ACLS certification
- CDS and DEA license
- Proficient computer skills
