About This Position
About the Role:
The SS Memory Care Coordinator plays a pivotal role in overseeing and enhancing the quality of care provided to residents with memory impairments within specialized care settings. This position is responsible for developing, implementing, and continuously improving individualized care plans that address the unique cognitive and emotional needs of each resident. The coordinator collaborates closely with interdisciplinary teams, families, and external healthcare providers to ensure a holistic approach to memory care. By monitoring resident progress and adapting strategies accordingly, the role aims to maximize residents' quality of life and safety. Ultimately, the SS Memory Care Coordinator serves as a key advocate and resource for both residents and staff, fostering an environment of compassion, respect, and clinical excellence.
Minimum Qualifications:
- Must possess a social services designee certificate or be willing to obtain one (Essential will assist)
- Minimum of 1 year experience working in memory care or geriatric healthcare settings.
- Strong knowledge of dementia-related conditions and evidence-based memory care practices.
- Excellent communication and interpersonal skills to effectively collaborate with multidisciplinary teams and families.
- Ability to manage multiple priorities and maintain detailed documentation in compliance with healthcare regulations.
Preferred Qualifications:
- Certification in Alzheimer’s Disease and Dementia Care or equivalent specialized training.
- Advanced degree in a relevant healthcare discipline.
- Previous leadership or coordinator experience in a long-term care or assisted living environment.
- Familiarity with state and federal regulations governing memory care facilities.
- Must be willing to work Full Time
Responsibilities:
- Develop and manage personalized care plans tailored to residents with memory-related conditions such as Alzheimer's disease and other dementias.
- Coordinate and lead interdisciplinary team meetings to review resident progress and update care strategies.
- Provide training and support to staff on best practices in memory care and behavioral management techniques.
- Serve as a liaison between families, healthcare providers, and the care team to ensure clear communication and alignment on care goals.
- Monitor compliance with regulatory standards and implement quality improvement initiatives related to memory care services.
- Developing activity schedule/calendar for memory care unit
- Assist with admission process on memory care
- Complete monthly family calls/updates
- Assess referrals for appropriateness for unit
- Provide tours of unit
- Facilitating memory care unit programming
- Assist with social services portion of MDS assessments
