RN Primary Care Case Manager- Blueprint in Bennington , Vermont at Southwestern Vermont Health Care

The RN Primary Care Case Manager is responsible for providing case management services in primary care practices.

Requirements: Graduation from an accredited school of nursing and BSN or actively matriculating towards a Bachelor of Science in Nursing. Five years of clinical nursing experience required. Valid Vermont State Registered Nursing license. Valid State driver's license required. Upon conditional offer of employment, must be able to successfully pass Motor Vehicle Record Check in accordance with SVHC insurance requirements.
