Care Coordinator
Pay: $18.00 - $20.00 per hour
Job description:
Overview: Care Coordinators play an important role in the quality of care, as they serve
as the bridge between our providers, facility care teams, and patients/ patient families.
Responsibilities
● Work under the supervision of a Nurse Case Manager.
● Chart creation with data entry of patient demographics, medications, and diagnosis.
● Process and maintain records with compliance of HIPAA.
● Communicate daily with Providers, Nurse Case Managers, and other
interdisciplinary team members involved in the patient's care.
● Provide timely communication and follow-up with patients, families, and
healthcare providers regarding changes in patient condition.
● Conduct monthly wellness calls to patients. Provide kind and professional
support to patients and families.
● Facilitate patient access to appropriate medical and specialty providers as well
as other care coordination team support.
● Other duties as assigned.
