This position reports to the Case Management Director.

DEFINITION OF POSITION
-
The Patient Support Specialist in Case Management provides essential administrative and operational support to
the Case Management department, working directly alongside RN Case Managers and Social Workers. This
position facilitates the coordination of patient care from admission through discharge by managing documentation,
tracking authorizations, supporting interdisciplinary communication, and ensuring compliance with CMS
Conditions of Participation, HIPAA, and accreditation standards. The specialist plays a key role in removing
barriers to progression of care and ensuring accurate, timely information flows across all involved departments
and payers.

QUALIFICATIONS

Professional
-
High school diploma or equivalent required. Associate's or bachelor's degree in healthcare administration, health information management, or a related field preferred.

Preferred one year of experience in a healthcare setting; prior experience in case management, utilization review, or care coordination preferred.

Familiarity with hospital-based case management workflows, payer authorization processes, and discharge planning concepts.

Knowledge of medical terminology and insurance/payer terminology sufficient to support authorization tracking and payer correspondence.

Understanding of CMS Conditions of Participation related to discharge planning, patient rights, and required patient notices (e.g., Important Message from Medicare, MOON).

Strong verbal and written communication skills, with the ability to interface professionally with clinical staff, payers, patients, and families.

Excellent organizational skills with the ability to manage multiple concurrent tasks, track deadlines, and maintain accurate records.

Ability to work effectively in a fast-paced, team-oriented environment with frequent interruptions and shifting priorities.

Demonstrated commitment to patient confidentiality and privacy.
