Patient Scheduling & Care Coordination Specialist

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Patient Scheduling & Care Coordination Specialist based in Canada.

As a Patient Scheduling & Care Coordination Specialist, you’ll serve as a key point of contact for patients navigating a virtual healthcare program. You’ll manage appointment scheduling and rescheduling, coordinate telehealth logistics, and ensure patients have clear information at every step. The role combines patient-facing communication with detailed administrative coordination, making accuracy and empathy equally important. You’ll work closely with clinical and customer success teams to keep patient and provider workflows organized and efficient. Accountabilities:
  • Manage daily patient scheduling and rescheduling workflows across multiple platforms, ensuring appointments are accurately coordinated and updated.
  • Communicate directly with patients by phone, email, and text to coordinate appointments, answer questions, and provide clear guidance on next steps.
  • Support patients through appointment changes and telehealth logistics, ensuring they have the information and instructions needed to successfully attend virtual appointments.
  • Monitor appointment workflows proactively, identifying scheduling conflicts, gaps, or operational issues and addressing or escalating them as appropriate.
  • Maintain accurate and up-to-date patient and appointment information across internal systems and ensure scheduling procedures are consistently followed.
  • Coordinate closely with clinical and customer success teams to ensure patients are appropriately scheduled and receive a seamless experience.
  • Help maintain organized and efficient provider schedules by identifying opportunities to improve scheduling flow and capacity.
  • Provide a warm, professional, empathetic, and patient-centered experience during every interaction while supporting evolving administrative and patient experience needs.
Requirements:
  • Demonstrate exceptional attention to detail, strong organizational skills, and the ability to manage multiple tasks, priorities, and platforms simultaneously.
  • Bring strong interpersonal, written, and verbal communication skills, with a patient, empathetic, and professional approach to customer interactions.
  • Be comfortable supporting patients who may have questions, concerns, or difficulty navigating technology, while communicating clearly and reassuringly.
  • Have the ability to learn and work confidently across multiple software systems and adapt to new digital tools and workflows.
  • Demonstrate reliable follow-through and ownership, with the ability to manage tasks independently from start to finish.
  • Be proactive and able to recognize what needs to be done without requiring constant direction.
  • Thrive in a fast-paced, evolving environment and remain organized and dependable while priorities change.
  • Previous experience in healthcare, health-tech, telehealth, medical administration, or customer success is an asset.
  • Experience with patient scheduling, appointment coordination, electronic medical records, CRM systems, scheduling platforms, or telehealth software is considered a plus.
  • Familiarity with virtual healthcare workflows or experience working in a startup or rapidly changing environment is beneficial.
Benefits:
  • Fully remote position available anywhere in Canada.
  • Full-time employment at 40 hours per week, Monday through Friday.
  • 11:00 AM Eastern Time start, corresponding to approximately 8:00–9:00 AM local time depending on location and time of year.
