Description:

We are seeking a compassionate and dedicated Survivorship Nurse Practitioner to join our care team. This vital role involves providing specialized care and support to cancer survivors, helping them navigate challenges and improve their quality of life. In this role you will:

  • Build and leverage relationships founded on trust and integrity to facilitate opportunities,
  • Coordination with providers, venders, and ancillary support services to provide continuity of care,
  • Ensure specialty services to patients with cancer by proactively identifying patient centered opportunities to improve the patient and family's experience of their illness, anticipate and treat distressing symptoms.

Requirements:

Key Responsibilities

  • Provide evidence-based consultation care that improves the quality, efficiency, and effectiveness of needed health related interventions and resources for oncology patients,
  • Assess, diagnose, and implement high-quality psychological care for those patients going through active cancer treatment
  • Provide concurrent care to patients in conjunction with Oncologist, Palliative Care providers, and Natural Pathic providers in coordination of care,
  • Provide patient education on short-term and long-term side effects of both disease and treatments,
  • Assess and care as recommended by professional governing organizations within the profession,
  • Closely communicate and coordinate with referring offices to include a summary of care and care plan,
  • Closely communicates with the patients referring provider to make recommendations and discuss the ongoing plan of care that encourages adherence to recommend follow up appointments, labs, or imaging,
  • Actively participate in weekly chart round discussions,
  • Complete all necessary charting and billing as required,
  • Other duties as needed and directed.

  • Caring, compassion and a passion to help others,
  • Ability to utilize telemedicine to increase access to patients/families,
  • Public speaking to promote awareness of the value of incorporating the service into comprehensive cancer care

Education/Experience

  • Graduate of accredited Physician Assistant program
  • Licensed in the State of Alaska
  • Preferred, 3+ years’ experience with oncology
  • DEA license
