Summary:
The Licensed Practical Nurse (LPN) provides basic nursing care to residents under the supervision of a nursing leader. Responsible for nursing care according to physicians instructions in compliance with established nursing techniques, procedures, and administrative policies, and promoting a safe, clean, and supportive environment for residents.
Essential Functions
- Provide nursing care to residents according to physician orders and established procedures, including administering medications and treatments and documenting care accurately.
- Assess, observe, and report residents’ physical and mental conditions to appropriate nursing staff, participating in the development and implementation of individualized care plans.
- Assist residents with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, feeding, and mobility support (repositioning, turning, transferring, and ambulation).
- Measure, record, and monitor vital signs and other clinical indicators, ensuring timely and accurate documentation in electronic health records.
- Coach and support nursing assistants and ancillary staff, ensuring compliance with facility policies and quality standards.
- Maintain a safe, clean, and sanitary environment for residents, co-workers, and visitors, following infection control protocols and facility safety procedures.
- Respond promptly to emergencies and initiate appropriate life-saving measures when necessary.
- Provide emotional support, education, and guidance to residents, families, and staff while upholding resident rights and confidentiality.
- Participate in admissions, transfers, discharges, staff meetings, and in-service training as assigned to ensure continuity and quality of care.
- Comply with all company policies, procedures, and applicable laws, including HIPAA and the Code of Conduct, completing required training and reporting concerns promptly.
- This position is designated as essential to the operations of the organization. Team members in this role may be required to report into work (on-site at the community) during emergencies, inclement weather, or other situations when the organization remains operational.
Physical Demands
- Always required to stand, walk, and assist residents.
- Frequently required to lift and/or move up to 50 pounds.
- Frequently required to bend, reach, push, and pull.
- Occasionally required to lift or assist in lifting individuals over 100 pounds with team or equipment assistance.
- Occasionally required to kneel or crouch.
- Visual acuity to observe resident condition and read charts.

Work Environment
- Always works indoors in resident-living area.
- Frequently exposed to bodily fluids and communicable diseases; PPE provided.
- Frequently works near residents experiencing physical or emotional distress.
- Occasionally exposed to strong odors, cleaning chemicals, and medical waste.
- Occasionally works in noisy environments due to equipment and alarms.
- Rarely works outdoors or in non-clinical areas.
Qualification Requirements
- High school diploma or equivalent (GED) required.
- Graduate of accredited Practical Nursing Program, required.
- Actively registered with the North Carolina Board of Nursing, and in good standing, required.
- Current CPR certification (or ability to obtain within 30 days of hire).
Preferred Qualifications
- One year of relevant experience working in long-term care or assisted living, preferred.
Knowledge, Skills and Abilities
- Knowledge of basic resident care procedures and medical terminology.
- Understanding of infection control practices and hygiene protocols.
- Assisting with resident mobility, transfers, and use of adaptive equipment.
- Taking and recording vital signs and documenting care accurately.
- Respond calmly and effectively to emergencies or stressful situations.
- Communicate clearly with residents, families, and the care team.
- Maintain resident confidentiality and professional boundaries.
- Follow written and verbal instructions and adapt to changing priorities.
- Demonstrated compassion, patience, and attentiveness to resident needs.
Schedule:
- 12-hour shifts
- Night Shift, 7pm-7am available
- Day Shift, 7am-7pm available
- Weekend availability
