Position Summary:
This is an entry level management position responsible for the supervision and coordination of quality services and staff of a case management team focused on the provision of case management services in the areas of dependency and adoption.
Essential Functions:
- Responsible for direct supervision of case management team as well as coordination, accuracy, appropriateness, and timeliness of workflow.
- Must be available as mentor and coach to assist and train team members in the timely preparation and completion of work assignments.
- May carry out an occasional case troubleshoot and ensure timely monitoring and delivery of services.
- May require the use of your personally owned vehicle.
- Ability to document timely in a clear, concise manner, and teach others to do so.
- Possess skills to assist, interpret, and deliver case and court related information.
- Possess a positive attitude concerning all aspects of working in a challenging environment including significant patience and respect for peers and team members, as well as the children and families receiving program services.

Education & Experience:
Bachelor's degree in human services, master’s degree preferred. Case Management Certification required. Subject matter expertise in foster care and adoptions. 2+ years of supervisory experience and 2+ years in direct service preferred.
Specific job responsibilities and goals include but are not limited to:
- Supervise day to day operations of one case management team under the direction of Program Director and Assistant Program Director, Case Management Services.
- Oversee and direct the client data collection and reporting processes, ensuring that services are delivered as designed and outcome measures defined by the program are accomplished.
- Ensures of cases have a full case review supervision bimonthly and the case manager has individual supervision bimonthly. These will be alternated every other month, ensuring the case manager feels supported and mentored.
- Participate in projection tracker updates and permanency cohort meetings.
- Intentionally participates in the retention of staff, treating everyone based on their level of need.
- Coordinates team on-call cycle ensuring that clients have access to case management and other staff as appropriate.
- Handles low level crisis calls and situations, informing, and engaging chain-of-command as necessary and appropriate.
- Provides, as needed, assigns direct services to programs, children, and families.
- Participates in community groups, meetings, and training related to program issues YFA is involved in at the direction of Program Director and Assistant Program Director.
- Maintains professional certification by participating in continuing education process.
- Must understand and demonstrate sensitivity to cultural, community, family, and child needs.
- Completes other tasks or special projects as assigned.
- Participate in the on-call rotation.
- Other duties as assigned by immediate supervisor or leadership.
- Trained and authorized to access protected health information.
Physical Requirements:
Ability to travel regionally on a regular basis and statewide as needed; ability to bend, lift, and carry 30 pounds.
