Title: Case Manager
FLSA: Non-Exempt, full-time, yearlong (This position is expected to last for the duration of the grant of one year.)
Pay: $26.25 hourly


About the Mobile Immigration Service Program
The Mobile Immigration Service Program is a grant in partnership with Esperanza Immigrants Right Project and Asian Americans Advancing Justice. The funding will support the provision of mobile legal assistance and intensive case management services for newcomer and long-term migrants experiencing homelessness throughout Los Angeles County. This yearlong grant will integrate Esperanza’s legal expertise, Asian Americans legal and language support and with Amanecer’s case management to address the needs of unhoused migrants.

Amanecer will target client-centered needs, meeting clients where they are, whether in schools, parks, or their homes. The Response Team will address urgent needs and connect clients to essential resources such as food, housing, and financial assistance.


Position Summary
The Case Manager works directly under a licensed clinician and case manager supervisor and is responsible for ensuring that clients are linked to needed services in their community, through linkage, consultation and placement. The Case Manager must be able to work effectively with a range of clients and staff from diverse ethnic and cultural backgrounds, as well as with private and public agencies.

The Case Manager provides mental health rehabilitation and case management/broker services for eligible clients (medical, educational, social, prevocational, vocational rehabilitative or other needed community services.). The Case Manager is knowledgeable in housing support. They collaborate with the therapist when needed, facilitating communication and translation in a culturally sensitive, trauma-informed manner.

Minimum Qualifications & Competencies
Education /Background
:

  • BS/BA degree highly preferred, but equivalent combinations of education and experience may be considered to meet qualifications for this position.
  • 2 years’ experience in case management preferred.
  • Must be able to meet and/or transport clients in the community as needed per program requirements.
  • CA Driver License required.
  • Bilingual Spanish speaking required.

Other Requirements:

  • Knowledge of community health care and community services.
  • Knowledge of case management methods, procedures, and documentation.
  • Experience working with multi-ethnic/racial clients and communities.
  • Ability to adhere to agency and clinic policy and procedures.
  • Ability to maintain confidentiality of records and information.
  • Ability to work well as part of the case manager team.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to prepare effective written materials, including correspondence and reports.
  • Ability to be flexible and multitask, handling several tasks simultaneously.
  • Ability to stay focused on duties and projects and get them completed on deadline as assigned.
  • Proficiency at computer required.

Hybrid Remote and Field-Based: Services are provided primarily in the community where clients are more comfortable in order to meet treatment needs and goals most effectively. Transportation and community outreach is expected when appropriate/needed. Position is hybrid remote and requires telehealth and in-person appointments in the field and in the office, meeting with clients, prospects and community partners as needed. Position requires transporting clients as needed. Employee must be available to work during office hours.
