TITLE: Case Manager & Peer Advocate
FLSA: Non-Exempt
PAY: $26.25 hourly, plus benefits, mileage reimbursement
SCHEDULE: Monday-Friday plus on-call weekends as scheduled as part of a team, which may include holidays (overtime and holiday pay)
POSITION SUMMARY
The Case Manager & Peer Advocate provides comprehensive and intensive case management services to children and adults with severe and persistent mental illness who may have history of homelessness and/or history of hospitalizations. Case management services include linkage to community resources such as housing, medical, educational, and vocational services. In addition, the Case Manager & Peer Advocate provides rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals.
As part of the case management team, this role as an advocate for clients and families. They make an impact in a variety of ways, including, but not limited to:
- Providing targeted case management and mental health rehabilitation services, including linkage to community resources, money management support, skills training, and independent living skills development;
- Assisting clients who may be experiencing homelessness in obtaining resources and applying for housing;
- Involving clients in establishing goals in the following areas: living arrangements, social relationships, constructive use of leisure time, productive activity, medical care, and others;
- Facilitating self-help groups and peer-to-peer/family-to-family/parent-to-parent groups;
- Transporting clients for appointments and other pre-approved outings;
- Providing on-call crisis support 24 hours / 5 days a week by phone or in-person if needed;
- Participating in the on-call weekend rotation to provide crisis support, which may include holidays.
This role supports clients in our FSP and OCS Programs:
- About Full-Service Partnerships (FSP): FSP is designed for adults ages 26-59 who have been diagnosed with a severe mental illness and would benefit from an intensive service program. The foundation of Full-Service Partnership is doing “whatever it takes” to help individuals on their path to recovery and wellness. FSP embraces client-driven services and the ability for each person to choose services based on their individual needs. Unique to FSP programs are a low staff-to-client ratio and crisis support.
- About Outpatient Care Service (OCS): OCS employs a multi-disciplinary treatment team that brings intensive services to a location convenient to those clients that can’t or won’t receive services in traditional mental health settings. Services are provided in locations in the community where the client feels comfortable. OCS supports children and adults and their families.
MINIMUM QUALIFICATIONS AND COMPETENCIES
Education /Background:
- BS/BA degree in social work-related field is highly preferred.Equivalent combinations of education and experience may be considered.
- 2 years’ experience in case management required.
- Lived mental health experience preferred.
- CA Driver License required and clean driving record for insurance in order to provide transportation to clients for appointments and other pre-approved outings.
- Bilingual Spanish speaking required.
Other Competencies:
- Knowledge of community healthcare and community services.
- Knowledge of case management methods, procedures, and documentation.
- Experience working with multiethnic/multiracial clients and communities.
- Experience with HIPPA, maintaining client confidentiality.
- Ability to work well as part of the case manager team.
- Effective communication skills (in-person, virtual, in writing, reports, documentation, one-on-one, in groups).
- Proven time-management skills.
- Proficiency at computer required.
HYBRID REMOTE
Position is hybrid remote and requires telehealth and in-person appointments in the field and in the office, meeting with clients, prospects and community partners as needed. Employee must be available to work during office hours. Employees in crisis programs must be available to participate in the on-call rotation schedule as assigned.
