Position Overview

We are seeking an experienced Home Health Occupational Therapist to join our dynamic team. The ideal candidate will be dedicated to improving the quality of life for our patients by providing expert occupational therapy services tailored to individual needs. This role offers the opportunity to work autonomously while collaborating with interdisciplinary teams to ensure the highest standard of care.

Responsibilities
  • Conduct comprehensive assessments of patients' physical and functional abilities.

  • Develop personalized treatment plans in accordance with physician recommendations and patients’ goals.

  • Provide skilled occupational therapy interventions to improve mobility, strength, balance, and overall function.

  • Educate patients and caregivers on therapeutic exercises, safety, and injury prevention.

  • Monitor progress and adjust treatment plans as needed to achieve optimal outcomes.

  • Accurately document clinical findings, interventions, and patient responses in a timely manner.

  • Collaborate with physicians, nurses, and healthcare professionals to ensure coordinated care.

  • Maintain compliance with HIPAA, Medicare, and other regulatory standards.

  • Participate in professional development and stay current with best practices.

  • Supervise Certified Occupational Therapy Assistants (COTAs) as needed.

Qualifications
  • Graduate of an accredited Occupational Therapy program with a current license to practice in Washington, DC.

  • Minimum of 3 years of clinical experience as an Occupational Therapist; home health or SNF experience preferred.

  • Strong clinical reasoning, problem-solving, and assessment skills.

  • Proven ability to adapt treatment plans to meet diverse patient needs.

  • Confidence in recognizing and appropriately responding to medical emergencies.

  • Excellent communication and interpersonal skills for patient, caregiver, and team collaboration.

  • Proficiency with EMR systems (Kinnser WellSky or Homecare Homebase preferred).

  • Reliable transportation and a valid driver’s license.
