Job Description

Nurse Practitioner – Primary Care (Full Time – Permanent)


The Role and Opportunity The Primary Health Care Nurse Practitioner plays a key role in providing comprehensive primary health care services to individuals accessing care through the CMHA Nurse Practitioner Clinic. Services are delivered in the clinic setting and, when required, in the community. The Nurse Practitioner provides holistic, client-centred care through health assessment, diagnosis, treatment, and follow-up across the lifespan. The role also includes health promotion, chronic disease management, collaboration with inter-professional team members, and participation in program development and quality improvement activities.


Salary: $107,812 – $114,600

Hours of Work: 35 per week

Responsibilities

1.Program Development:

Participate in program development and policy-making activities that influence health services and practices in Primary Health Care at CMHA.Collaborate with all members of the inter-professional team to plan and deliver holistic, comprehensive, patient- and family-centred care.Contribute to the dissemination of new knowledge and best practices throughout the organization.Act as a mentor and clinical practice coach, advancing nursing skills and serving as a role model for nursing colleagues and other members of the inter-professional team.
2.Service Care Planning:

Conduct comprehensive, ongoing health assessments, including physical, psychosocial, and environmental factors, order and interpret all laboratory tests and develop individualized care plans.Diagnose and manage acute and chronic health conditions using evidence-based approaches within the Nurse Practitioner scope of practice.Ensure clients’ linguistic, cultural, and social needs are incorporated into care planning and service delivery.Demonstrate accountability and professional responsibility in the delivery of high-quality primary health care services.Ensure care plans are current, documented, and implemented collaboratively with clients and team members.Initiate appropriate consultations, referrals, and care coordination with internal and external health care providers.Participate in the evaluation and improvement of models of care, service processes, and care delivery outcomes.Communicate effectively and respectfully with clients, families, and team members to support positive health outcomes and client engagement.3.Health Promotion and Disease Prevention:

Prescribe medications, order diagnostic tests, and initiate treatments and therapeutic interventions in accordance with legislative authority and best practice guidelines.Provide health education related to disease prevention, chronic disease management, lifestyle modification, and wellness promotion.Support clients in navigating the health system and accessing appropriate community and social supports.Incorporate current research evidence and clinical guidelines into clinical decision-making and care planning.4.Family Education and Support:

Educate the family and substitute decision maker on the nature of the illness, treatment, medication and community integration.Provide resources and support to help families and substitute decision maker cope more effectively.Provide culturally appropriate resources and support to help families and substitute decision makers cope more effectively
5. Linking with Community Resources:

Develop and maintain effective working relationships with community agencies, primary care providers, hospitals, and social service organizations.Support clients in accessing and coordinating services across the health and social care system.Prepare clients and external providers for transitions of care and shared service delivery expectations.6.Advocacy:

Advocate on behalf of the client as necessary.Present the services of CMHA Metro Toronto in a professional manner.7.Discharge Planning:

Participate in the development of a discharge plan in collaboration with the client, Primary Health CareTeam and relevant others.Ensure that continuity of care is provided where necessary.8.Administration

Maintain timesheets, record keeping (i.e. Kardex, biopsychosocial, daily notes and assessments) within set timelines.Maintain clear, accurate, and appropriate records of all patient care deliveredMaintain relevant clinical document sin accordance with the College of Nurse of Ontario.Maintain membership with the College of Nurses of Ontario and NPAO.Participate on agency-wide client safety and continuous quality improvement (CQI) initiatives which include but may not be limited to accreditation and health and safety related activities.Work collaboratively and contribute to the implementation of integrated client safety initiatives, programs and processes
Experience & Education Required

Education

BScN Degree and Registration with the College of Nurses of Ontario and NPAO


Related Experience:

Experience in providing counselling to clients around issues related to medications and their effectsMinimum 5 years of related clinical practice in community mental health and primary health care setting.Experience providing a consultative/educative service from a nursing perspective to other service providers and other community agency staffSkills Required
