Job Description
As a Case Manager in JASA’s Community Guardian Program (CGP), you’ll support adults who have been determined by the courts to be legally incapacitated and require guardianship services. In this role, you’ll provide case management, advocacy, crisis intervention, and service coordination to help clients maintain their health, safety, and quality of life.
Shift Schedule:Monday–Friday: 9:00 AM – 5:00 PM
Hours Per Week:35
Location:This position requires two (2) days per week in JASA’s Manhattan office and a minimum of three (3) days per week traveling throughout Queens, Manhattan, and Brooklyn to conduct client visits. Additional travel may be required during emergency situations.

As a Case Manager, you’ll provide comprehensive case management services to vulnerable adults under court-appointed guardianship. You’ll assess client needs, coordinate services, advocate for client well-being, and ensure compliance with court-ordered requirements.
Case Management & Client Support- Conduct initial assessments and monthly client visits.
- Develop and implement individualized service plans.
- Coordinate services including healthcare, benefits, housing, legal assistance, and home care.
- Respond to crisis situations and provide appropriate interventions.
- Ensure court-ordered services are in place and meeting client needs.
- Advocate on behalf of clients and their best interests.
- Build and maintain relationships with community providers and service agencies.
- Participate in case consultations, meetings, and trainings.
- Maintain accurate case records and reports.
- Complete required documentation within established timeframes.
- Utilize agency software and mobile technology to manage client services.
- Perform additional duties as assigned.
- Bachelor’s degree in Social Work, Sociology, Psychology, Human Services, or a related field required.
- One (1) year of case management experience preferred.
- Strong organizational and time management skills.
- Ability to travel independently to client homes using public transportation.
- Strong communication and documentation skills.
- Computer proficiency and ability to learn new systems.
- Ability to work effectively with individuals from diverse backgrounds.
