Salary Range: $43,924 - $45,461

This position is Monday - Friday, 8:30- 4:30 PM or 9:00-5:00 PM and located at the Stephen Klein Wellness Center, 2144 Cecil B. Moore, Philadelphia, PA.
What You'll Do
As a Referral Specialist, you will play a key role in ensuring patients receive timely, coordinated care.
- Manage referrals from provider orders through specialist follow-up, ensuring continuity of care
- Assist uninsured or underinsured patients with insurance applications and financial documentation
- Track and follow up on hospital admissions and referrals to support patient outcomes
- Answer incoming calls, schedule appointments, and support front desk operations as needed
- Provide clerical and administrative support to ensure a smooth clinic workflow
- Collaborate with care teams to meet Patient-Centered Medical Home standards

What You Bring
- High School Diploma or GED required
- At least two years of experience in healthcare administration, referrals, patient services, insurance verification, care coordination, or a similar role
- Experience using an Electronic Health/Medical Record (EHR)/(EMR), Practice Management System (eClinicalworks is a plus)
- Strong organizational skills and attention to detail
- Excellent verbal and written communication skills
- Professional telephone etiquette and customer service skills
- Ability to manage multiple priorities in a fast-paced environment
- Ability to work independently while collaborating effectively with a team
- Experience working with underserved populations is highly valued
- Bilingual Spanish/English is a plus
