Job Description

This position provides direct and indirect patient care under the direction of a registered nurse. Assists patients with activities of daily living (ADL's), provides for personal care, emotional support and performs more complex clinical skills under the direction of a registered nurse.

Under the supervision of the charge nurse, assists in planning, organizing, implementing, and evaluating activities occurring in the nursing station by performing clerical and receptionist duties and maintaining the physical environment of the area.

MINIMUM QUALIFICATIONS

1. High School Graduate or GED equivalent from a department of education accredited program.

2. Current State of Florida certification as a certified nursing assistant

3. Demonstrated proficiency in clinical knowledge and skills pertaining to patient care areas when in CNA/Unit Secretary role.

4. Demonstrated computer skills to include proficiency in the use of an electronic health record, CPSI, E Power Doc, Microsoft Word, Excel and the use of web-based programs.

5. Current BLS certification required. Certifications are only accepted from The American Heart

Association.

ADDITIONAL QUALIFICATIONS BASED ON TIER STATUS

1. Teir 1 to include all qualifications, requirements, and job duties described in this policy for Certified Nursing Assistant/Unit Secretary in the ED ONLY.

2. Teir 2 to include EITHER certification in ECG Monitor Tech OR Certified Phlebotomy Tech AND the job duties described in this policy for Certified Nursing Assistant/Unjt Secretary in the ED AND EITHER the duties described in this policy for ECG Monitor Tech OR Certified Phlebotomist.

3. Teir 3 to include BOTH certification in ECG Monitor Tech AND Certified Phlebotomy ALONG with the duties described in this policy for Certified Nursing Assistant/Unit Secretary in the ED and the duties described in this policy for Certification in ECG Monitoring AND Certified Phlebotomist.

LANGUAGE SKILLS

LIMITATIONS/HAZARDS

1. General work area chemicals as listed on the departmental chemical inventory.

2. May be exposed to chemical toxins during medication administration and radioactive materials during imaging procedures.

3. OSHA Category I exposure to infectious diseases and blood and body fluids.

4. May be exposed to combative, agitated, or violent patients.

5. Occasionally subjected to extended or irregular hours

6. Occasional pressure due to unpredictable situations, emergency, or crisis situations, or due to multiple calls and inquiries

PHYSICAL REQUIREMENTS

Essential Technical/Motor Skills: Physical ability for:

1. Extensive; walking and standing; kneeling; squatting.

POSITION ACCOUNTABILITIES (Criteria indicated on Performance Evaluation)

1. Job Duties (Statements Pertinent to position and tier status).

1.1 Certified Nursing Assistant

1.1.1 Serves and removes food trays and assists with meals or feeds patients if necessary. Distributes drinks and snacks to patients as applicable.

1.1.2 Prepares and assists with procedures related to patient admissions, discharges, and transfers.

1.1.3 Observes patients and reports any changes in behavior or condition to RN or Providers.

1.1.4 Provides patient care which may include bed bath, oral hygiene, combing hair, back care, dressing patients, and other general care as necessary throughout the patient stay in ED.

1.1.5 Provides general nursing care such as positioning patients, lifting and turning patients, applying/utilizing special equipment, assisting in use of bedpan, commode, and ambulating patients.

1.1.6 Answers patients' call lights, anticipates patients' needs and makes rounds of patients.

1.1.7 Takes and records temperature, pulse, respiration, weight, blood pressure, intake-output, and ADL's. Report any abnormal findings to Registered Nurse.

1.1.8 Report patient complaint of pain to patient's nurse.

1.1.9 Observe patient's mental and physical conditions, report any changes to patient's nurse.

1.1.10 Performs routine treatment procedures including enemas, oral suctioning, application of appliances for heat and cold, sitz baths, postmortem care, and assists staff in complicated treatments or procedures.

1.1.11 Will be knowledgeable of sterile techniques and infection control procedures. Will clean and turn over room with proper technique.

1.1.12 Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

1.1.13 Collects and transports specimens according to hospital policy and procedure.

1.1.14 Assists patients with handling and caring of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids, prosthetic devices, and valuables) throughout their hospital visit.

1.1.15 Assists in maintaining a safe and clean environment by cleaning utility rooms, cleaning equipment, and preparing for disinfection or sterilization.

1.1.16 Manages and operates equipment safely and correctly; and promptly reports malfunctioning/broken equipment. Maintain bulletin boards with correct, updated metrics.

1.1.17 Ensure that patient is properly identified by following established policy before care is performed.

1.1.18 Will perform waive testing as appropriate.

1.1.19 Will fill-out EMTALA form completely and accurately. Will complete Transfer Chart Audit reports.

1.1.20 Performs patient care and communications within the guidelines of the Relationship and Results Oriented Healthcare Model utilizing the recommended bundles of best practice.

1.1.21 Communicates and completes handoff documentation for effective handoff of patients care (SBAR) for admission or transfer to another health care facility.

1.1.22 Demonstrates understanding of legal issues in the Emergency Room Department (i.e. child abuse, rape, blood alcohol, gunshot wound, medical examiner, Baker Act, Marchman Act, human trafficking and elder abuse.

1.1.23 Assists with securing patient valuables by labeling personal items and storing dentures, glasses, etc., in a safe place and ensuring that valuables stay with the patient when transferred/discharged. Encourages family member to take valuables/belongings home that are not necessary for the patients while in the hospital.

1.1.24 Utilizes and ensures staff that uses supplies efficient, following infection control procedures, avoiding waste and follows patient charging procedures.

1.2 Unit Secretary

1.2.1 Maintains the physical environment of the nurses' station and surrounding areas in a clean and organized manner.

1.2.2 Fax doctor's orders to appropriate areas under the supervision of a registered nurse; confers with charge nurse regarding scheduling patient's activities and tests. Fax and/or scan documents to medical records appropriately.

1.2.3 Answers telephone and call system using proper telephone etiquette, takes messages, and communicates messages to the appropriate person.

1.2.4 Orders supplies and equipment, receives and places items in their appropriate storage areas.

1.2.5 Demonstrates a thorough working knowledge of medical and pharmaceutical terminology.

1.2.6 Communication: RN, doctor and unit information is disseminated appropriately

1.2.7 Utilizes call system for patient communications effectively and courteously.

1.2.8 Ensure that all records are completed prior to discharge of the patient.

1.3 Certified ECG Monitor Technician

1.3.1 Along with the duties described in original Job Description for the CNA/Unit Secretary in the ED. This employee has gained certification in monitoring ECG rhythms.

1.3.2 Maintain certification on file with the organization.

1.3.3 Practice within the scope of your duties as a monitor technician.

1.3.4 Monitor patients placed on ECG Monitors within the department.

1.3.5 Recognize and report abnormal rhythms to the appropriate person.

1.3.6 Recognize lethal rhythms and call the appropriate code responses or notifications to the care team.

1.3.7 Place patients on ECG Monitoring when delegated to do so.

1.3.8 Transport patients between departments while monitoring ECG Rhythm

1.4 Certified Phlebotomy. Technician

1.4.2 Along with the duties described in original Job Description for the CNA/Unit Secretary in the ED. This employee has gained licensure to practice as a Phlebotomy Technician in the ED.

1.4.3 Maintain up to date licensure on file with the organization.

1.4.4 Have on-the-job training with the lab department for competency assessments.

1.4.5 Practice within the scope of a CPT per State Guidelines

1.4.6 Obtain blood samples/draws on patients in the ED by collection within the practice standards.

1.4.7 Educate patients on the procedure.

1.4.8 Hand Hygiene in accordance with organization policy

1.4.9 Deliver blood samples to the laboratory for processing.

2. Compliance/Financial.

2.1 Supports HRMC Mission Statement and follows the Medical Center's Compliance Program and Ethical Standards of Conduct.

2.2 Supports Departmental Policies related to compliance and completes documentation in observance of regulatory Standards.

2.3 Attends training and in-services concerning all aspects of assigned duties. Responsibilities include, but are not limited to, federal and state statutes, regulations and guidelines.

2.4 Reports any incidences of suspected fraud, waste, or abuse violations to the Compliance Officer.

2.5 Practices careful, efficient, and non-wasteful use of supplies and linen.

2.6 Utilizes equipment in a responsible manner.

2.7 Follows established charge procedure for patient charge items.

2.8 Utilizes PTO/DTO system appropriately.

2.9 Employees will review and revise their hospital email regularly for important notices and/or assigned competencies.

3. Communication/lnterpersonal Skills (Statements Pertinent to position).

3.1 Communicates in a clear and concise manner to the team leader related to patient care.

3.2 Demonstrates knowledge of nursing and administrative hierarchical structure and chain of command. Follows proper lines of communication.

3.3 Reports any change observed in the condition of behavior of patient and unusual incidents.

3.4 Establishes and maintains interpersonal relationships with patients, visitors and other hospital personnel while ensuring confidentiality of patient information.

3.5 Documentation meets current standards and policies.

3.6 Attends and participates in nursing department meetings or committees as assigned.

3.7 Hospital Email must be checked weekly. PRN staff must check their hospital email when they return to duty.

3.8 Refers to RN all calls regarding critical lab values.

3.9 Refers to the RN all nurse calls regarding request for health information.

3.10 Reports off to the oncoming Nurse Supervisor or the Charge Nurse the status of pending transfers or other incomplete duties.

Quality Improvement.

4.1 Participates in Nursing and Hospital TQM projects as needed.

4.2 Records, collects, and correlates information for CQI continuous monitors or TQM projects assigned.

4.3 Participates in a just culture that holds organizations accountable for the systems they design and for how they respond to staff behaviors fairly and justly.

4.4 Follows the National Patient Safety Goals.

4.5 Follows HRMC Patient Safety Initiatives.

5. Age Specific Competency

This staff member must be able to demonstrate the knowledge and skill necessary to provide care, based on physical, psychosocial, educational and safety related criteria, appropriate to the age of the patients served in his/her assigned service area. The skills and knowledge needed to provide such care may be gained through education, training, and/or experience.

6.1 Participates in relevant continuing education and educational offerings relevant to position.

6.2 Annual in-service updates as required by hospital policy are completed. All required certifications remain up to date.
