As a Patient Access Specialist at Emmanuel Hospice, you will experience a rewarding work life while contributing to our team in a full time, hourly, non-exempt position.
Responsibilities:
- Processes all intake phone calls, electronic referrals, and walk-in requests for information about Emmanuel Hospice care and services. Manages the referral process from initial communication to admission through coordination with patients, families, facility staff, doctors, and partners. Gathers patient information from various sources, enters referral data into the electronic medical record, and develops hard chart copies.Obtains physicians orders and medical records to support hospice services. Verification of insurance and hospice benefit information. Schedules appropriate staff to conduct informational/sign-on visits and nursing admission visits. Builds relationships with referral sources as part of the Business Development team.Coordination with complementary services team members (massage therapy, music therapy). Assists with multi-line phone call management. Communicates with patients’ interdisciplinary teams in an organized and timely manner. Provides a calm, professional, and pleasant experience in person and over the phone for fellow staff, patients/families, facilities, vendors, and partners. Performs other responsibilities as requested/assigned by the Director of Business Development

Qualifications:
- High School Diploma required, some college preferredPrevious healthcare experience required, hospice experience preferredExperience in Microsoft Office products preferredAbility to read and interpret medical recordsPositive, professional attitude with clear communication and customer service skills
