What You’ll Do

As a Stroke Patient Navigator, you’ll report to the Director, Cardiovascular and Stroke Services. Participates in designing compassionate, timely, comprehensive, patient-centered care for the stroke patient. Provides patient, family, community, and professional education. Coordinates work to achieve and maintain Stroke Center designation from an accrediting body. Collects data to achieve and maintain AHA Get with the Guidelines for Stroke designation. Works closely with physicians to meet the needs of the stroke patients and the organization. Coordinates research data, community physicians, and Beacon Health System resources into a seamless model of access and care that benefits patients, participating physicians, and family members.

Stroke Patient Navigator Job Responsibilities

• Coordinate and oversee the Stroke Program to ensure high-quality, patient-centered care and compliance with regulatory standards.
• Monitor and manage stroke patients throughout hospitalization and post-acute care transitions.
• Lead Code Stroke responses and support timely, evidence-based stroke interventions.
• Collaborate with physicians, nursing, EMS, and interdisciplinary teams to optimize stroke care delivery.
• Develop, implement, and maintain stroke-related policies, protocols, clinical pathways, and patient education resources.
• Support Joint Commission stroke certification efforts and maintain program readiness.
• Track, analyze, and report stroke quality metrics, patient outcomes, and performance improvement initiatives.
• Conduct chart reviews, audits, and case reviews to ensure compliance with stroke standards and best practices.
• Educate patients, families, staff, and community members on stroke prevention, treatment, and recovery.
• Coordinate community outreach programs, health screenings, and stroke awareness initiatives.
• Participate in interdisciplinary rounds, tele-neurology consultations, and care coordination activities.
• Assist with budget planning, special projects, and other duties as assigned.

What You Bring:

As a Stroke Patient Navigator, this position requires comprehensive knowledge of nursing clinical care practices, procedures, and techniques, with a focus on neuroscience and/or trauma care. The successful candidate must possess strong analytical, problem-solving, organizational, and time management skills, with the ability to prioritize responsibilities and perform effectively in a fast-paced environment. Excellent verbal, written, and interpersonal communication skills are essential to effectively counsel patients and families while fostering positive relationships with patients, physicians, staff, and the public. The role also requires empathy, professionalism, and tact when working with patients and their families, as well as proficiency in computer systems and applications that support patient care and operational functions

Education and Experience

• The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of an accredited nursing program.
• A current license to practice as an RN in Indiana is required.
• A bachelor's degree in nursing is required.
• Prior experience managing and/or coordinating the care of neuro or stroke population is desirable.
• Prior clinical experience on a stroke unit or clinical unit that manages the care of stroke patients.
