ABOUT TRENTON HEALTH TEAM

Trenton Health Team (THT) fosters cross-sector collaboration and implements innovative projects to improve health and well-being in the greater Trenton community. THT operates with a broad understanding of what health, health equity, and well-being mean for the Trenton region. Our work shifts in response to the needs of our community and of our partners and is based on robust data analysis. Current priorities include chronic disease management, care management for community members with complex health needs, food and housing security, maternal and child health, mental health and substance use disorder, disability services, and neighborhood revitalization. Work in these areas is carried out with a wide range of stakeholders from across the region and across sectors, including those from the more than 100 organizations that participate in our Community Advisory Board.

POSITION OVERVIEW

The Program Manager is responsible for planning, implementing and reporting on a portfolio of projects focused on improving health and well-being in Trenton and the surrounding region. Reporting to the Director of Programs, the Program Manager will work to ensure that projects achieve deliverables, key partnerships are developed and sustained, budgets are maintained, and new strategies are developed to address emerging opportunities and needs in the community. Major functions include:

  • Project Management: Act as a project lead on one or more projects of moderate size, complexity, and scope, and serve on one or more project teams. Design and execute projects to ensure that all deliverables are met in a timely manner.

  • Team Leadership: Lead cross-functional project teams and directly supervise up to three staff.

  • Stakeholder Engagement: Develop and manage relationships with partners related to projects in the portfolio.

  • Budget Management: Manage project budgets to ensure accurate expenditures and compliance with funding requirements.

  • Strategy Development: Partner with leadership and external stakeholders to design, develop, and deploy strategies to improve health and well-being in Trenton and the surrounding region.

ESSENTIAL FUNCTIONS:

  1. Project Management: Serve as project lead on a selected number of projects to include:

  • Team Coordination: Convene project teams consisting of THT staff and external partners; clearly define and assign roles and responsibilities.

  • Project Management: Develop and maintain project charters, work breakdown structures, and other tools in consultation with the Project Supervisor. Manage team workflows to ensure timely progress toward milestones, deliverables, and KPIs. Actively consult with the Project Supervisor regarding project status or roadblocks.

  • Metrics & Outcomes: Partner with the project team to define, develop, and implement systems to track and measure project outcomes and metrics. Develop and implement quality improvement and assurance activities.

  • Documentation: Create and maintain project implementation folders within the shared drive, ensuring all drafts, final products, and relevant documents are organized and securely stored.

  • Communication: Facilitate internal team communications and lead productive meetings. Present about projects to external audiences.

  1. Stakeholder Engagement:

  • Participate in stakeholder meetings on behalf of THT.

  • Attend, organize, and facilitate stakeholders meetings and events to build relationships.

  • Keep abreast of community news and identify barriers or facilitators to programs.

  1. Budget Management:

  • Monitor spending on projects and approve monthly spending and variance reports.

  1. Strategy Development:

  • Contribute to the development of THT Strategy and support overall strategy development within the assigned portfolio of work.

  • Contribute to development of grant applications and other funding proposals

  • Develop the necessary subject-matter expertise to effectively lead assigned projects.

ADDITIONAL FUNCTIONS:

  • Support other THT programs and activities as needed.

  • Provide periodic verbal and written reports regarding the status and progress of work.

  • Participate in and contribute to regular team meetings.

  • Assume all other duties and responsibilities as assigned by the Director of Programs.

SUPERVISORY RESPONSIBILITIES:

  • Hire and supervise staff if/when needed

  • Recruit and supervise interns

REQUIRED QUALIFICATIONS

  • 3+ years of experience and Bachelor’s degree, or equivalent combination of education and experience.

  • Professional experience in community-based work (including outreach and engagement) and/or public health or clinical operations or administration.

  • Strong organizational, problem-solving, and analytical skills; able to manage priorities and multiple deadlines.

  • Commitment to serving low-income communities.

  • Versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm.

  • Excellent interpersonal skills with ability to communicate and interact effectively with individuals from diverse groups regardless of race, sex, age, and/or sexual orientation and members of underserved communities.

  • Excellent presentation skills with an ability to motivate and energize community members/groups to action.

  • Demonstrates excellent written and verbal communication skills with the ability to communicate across all strata of the community.

  • Excels working in a dynamic environment.

PREFERRED QUALIFICATIONS

  • Master’s degree in healthcare administration, public health, public administration, or a related field.

  • Experience working in Trenton communities.

  • Bilingual in written and spoken English and Spanish.

COMPUTER SKILLS

  • Excellent computer skills with specific knowledge of Google Workspace, Microsoft Office programs, and web-browsers/web-based programs; Experience with Salesforce, Constant Contact or similar tools is preferred.
