Position Summary

The Functional Wellness Registered Nurse is a key clinical and operational member of the Functional Wellness team, supporting patients throughout their wellness journey through a combination of telehealth care coordination, patient education, clinical follow-up, and in-person nursing services.

This role is designed for an experienced, highly organized RN who is comfortable working independently while maintaining close collaboration with prescribing providers and the broader clinical team. The Functional Wellness RN serves as an important connection between the patient and provider, helping ensure that treatment plans are understood, appropriately implemented, monitored, and documented.

The ideal candidate is passionate about preventative health, hormone optimization, metabolic wellness, weight management, healthy aging, and creating an elevated patient experience. This individual should be equally comfortable providing clinical education, reviewing a patient chart, coordinating laboratory testing, troubleshooting a medication question, and delivering an exceptional patient experience.

Essential Responsibilities

Patient Care & Telehealth Support

  • Serve as a primary clinical resource for patients enrolled in Functional Wellness programs.
  • Conduct RN-level telehealth and telephone follow-up within the RN scope of practice and established clinical protocols.
  • Provide ongoing patient education regarding prescribed treatment plans, medication administration, injection technique, laboratory testing, lifestyle recommendations, and expected follow-up.
  • Reinforce provider-developed treatment plans and ensure patients understand their medications, dosing instructions, monitoring requirements, and next steps.
  • Perform nursing assessments and identify symptoms, concerns, abnormal findings, or changes in patient condition requiring escalation to the prescribing provider.
  • Provide appropriate nursing triage and facilitate timely provider evaluation when concerns fall outside RN scope or established protocols.
  • Maintain appropriate professional boundaries between nursing education/support and medical diagnosis, prescribing, or modification of treatment plans.
  • Facilitate continuity of care throughout the patient's wellness journey.

Functional Wellness Program Support

Support patients participating in programs that may include:

  • Medical weight management and GLP-1 therapy
  • Hormone replacement therapy and hormone optimization
  • Testosterone replacement therapy
  • Thyroid optimization
  • Peptide and wellness therapies
  • Vitamin and nutrient optimization
  • Injectable wellness therapies
  • IV therapy
  • Metabolic health and longevity-focused programs
  • Other provider-directed functional and preventative wellness services

The RN is responsible for remaining knowledgeable regarding current Company protocols, treatment pathways, contraindications, monitoring requirements, and patient education standards associated with these programs.

Laboratory & Diagnostic Coordination

  • Coordinate laboratory orders and ensure patients understand appropriate timing and preparation requirements.
  • Track outstanding laboratory testing and assist with patient outreach when testing has not been completed.
  • Review incoming laboratory results according to established RN protocols and identify results requiring provider review.
  • Ensure abnormal, critical, or unexpected results are escalated appropriately and documented.
  • Coordinate repeat laboratory testing and follow-up appointments as ordered by the provider.
  • Assist in maintaining organized systems for laboratory monitoring and treatment-plan compliance.

Medication & Prescription Coordination

  • Support medication onboarding and patient education following provider evaluation and prescribing.
  • Educate patients regarding proper medication storage, administration, injection technique, supplies, and disposal.
  • Coordinate refill requests and route requests requiring clinical approval to the prescribing provider.
  • Track medication and pharmacy-related concerns and communicate with patients, providers, and pharmacies as appropriate.
  • Assist with prior authorizations and other clinical documentation when applicable.
  • Never independently initiate, discontinue, or alter a prescription or prescribed dose unless specifically permitted by applicable law, standing orders, and organizational policy.

Patient Communication & Retention

  • Provide timely, professional, and compassionate responses to clinical patient messages.
  • Proactively follow up with patients at appropriate intervals to evaluate adherence, tolerance, concerns, and overall experience.
  • Identify patients who are overdue for laboratory testing, follow-up appointments, or required clinical monitoring.
  • Help patients understand the importance of continued medical monitoring and appropriate follow-up.
  • Support patient retention through exceptional clinical communication, education, responsiveness, and continuity of care.
  • Identify opportunities to improve the patient's experience while maintaining appropriate clinical and ethical boundaries.

In-Person / Hybrid Responsibilities

Although a significant portion of this role may be completed remotely, the Functional Wellness RN will maintain an in-person clinical presence as scheduled or based on operational needs.

In-person responsibilities may include:

  • Patient intake and nursing assessments
  • Vital signs and clinical measurements
  • Medication and injection education
  • Subcutaneous and intramuscular injections
  • IV therapy when appropriately trained and credentialed
  • Blood draws/phlebotomy when applicable
  • Specimen collection
  • Patient onboarding
  • Clinical follow-up visits
  • Medication/supply education
  • Supporting providers during Functional Wellness clinic days
  • Other RN-level clinical services within scope of practice and Company protocols

Clinical Documentation & Quality Assurance

  • Complete accurate, thorough, and timely documentation for all patient interactions.
  • Ensure telephone calls, telehealth encounters, education, clinical concerns, provider communications, and follow-up activities are appropriately documented in the medical record.
  • Maintain patient privacy and HIPAA compliance in both remote and in-person work environments.
  • Participate in routine chart audits and quality assurance activities.
  • Identify gaps in documentation, follow-up, laboratory monitoring, medication management, or workflow and escalate concerns appropriately.
  • Assist in developing and improving Functional Wellness policies, protocols, patient education materials, and clinical workflows.
  • Participate in case review and quality improvement discussions with providers and clinical leadership.
  • Support adherence to applicable state Board of Nursing requirements, prescribing regulations, organizational policies, and standards of care.

Program Operations & Organization

The Functional Wellness RN plays an active role in maintaining an organized, scalable, and clinically sound wellness program. Responsibilities may include:

  • Maintaining patient tracking and follow-up systems.
  • Monitoring outstanding labs, appointments, refill requests, and clinical tasks.
  • Supporting medication and supply ordering workflows.
  • Assisting with inventory management for wellness medications and clinical supplies when applicable.
  • Communicating with pharmacies, laboratories, and other clinical partners.
  • Identifying workflow inefficiencies and recommending solutions.
  • Supporting new program launches, treatment offerings, and patient education initiatives.
  • Collaborating with administrative and clinical teams to create a seamless patient experience from initial inquiry through ongoing treatment.

Collaboration & Communication

  • Maintain consistent communication with prescribing providers regarding patient concerns, treatment tolerance, abnormal findings, and clinical questions.
  • Participate in scheduled Functional Wellness meetings and patient case reviews.
  • Work collaboratively with providers, nurses, administrative staff, patient coordinators, and leadership.
  • Demonstrate appropriate clinical judgment regarding when an issue can be managed through RN education and when provider evaluation is required.
  • Promote peer accountability through respectful communication, appropriate feedback, and collaborative problem-solving.
  • Help identify potential patient safety, compliance, workflow, or service concerns before they negatively impact the patient experience.

Patient Experience & Brand Standards

Every interaction should reflect an elevated, personalized, and medically responsible patient experience. The Functional Wellness RN is expected to combine clinical expertise with warmth, discretion, professionalism, responsiveness, and attention to detail.

Patients should feel that their care is highly coordinated and that the clinical team understands their history, goals, treatment plan, and next steps. The RN is responsible for helping create this continuity while maintaining appropriate clinical boundaries and ensuring that patient safety remains the priority.

Performance will be evaluated using both clinical quality and program-growth indicators, which may include:

The RN is expected to view these metrics not simply as performance measures, but as indicators of the health, quality, and continued growth of the Functional Wellness program.

Qualifications

Required:

  • Current, unrestricted Registered Nurse license in all applicable state(s)
  • Graduate of an accredited nursing program
  • Current BLS certification
  • Strong clinical assessment and nursing triage skills
  • Excellent written and verbal communication
  • Strong organizational and follow-through skills
  • Ability to work independently in a remote environment
  • High level of comfort with electronic medical records, telehealth platforms, and digital communication
  • Commitment to patient privacy and HIPAA compliance

Preferred:

  • BSN
  • Experience in functional medicine, wellness, primary care, endocrinology, weight management, women's health, men's health, urgent care, or medical aesthetics
  • Experience with GLP-1 medications, hormone replacement therapy, injectable medications, or IV therapy
  • Phlebotomy experience
  • Telehealth or remote patient-management experience
  • Experience working in a growing healthcare organization

Because this position operates in a hybrid environment, success requires exceptional communication and the ability to independently manage responsibilities without sacrificing collaboration or visibility.

The RN is expected to take ownership of assigned patients and workflows, communicate proactively when barriers arise, seek provider guidance when appropriate, and contribute positively to a culture of accountability, continuous improvement, and exceptional patient care.

Physical & Scheduling Requirements

This position utilizes a hybrid work model consisting of scheduled remote/telehealth responsibilities and designated in-person clinical responsibilities. The specific balance of remote and on-site work may change based on patient volume, clinical needs, program growth, and business operations.

Some evening or weekend availability may occasionally be required based on patient-care needs and program scheduling. Reliable internet access and an appropriate private workspace that allows for HIPAA-compliant remote patient communication are required when working remotely.

Scope of Role

Nothing within this job description authorizes the Registered Nurse to independently diagnose medical conditions, prescribe medications, modify provider-directed treatment plans, or perform functions outside the RN's legal scope of practice. All clinical responsibilities must be performed in accordance with applicable state law, Board of Nursing requirements, provider orders, organizational policies, and established clinical protocols.

  • Is a self-starter with drive, passion for aesthetics, and a friendly, outgoing personality
  • Ability to multi-task and present sound judgement
  • Strong math, analytical, and follow-up skills
  • Candidate must be able to work one weekend a month
