Hospitalist Care Coordinator in Pinehurst, North Carolina at FirstHealth of the Carolinas

RN or LPN to perform Hospitalist administrative work which includes:

Managing incoming calls for the Physicians from pharmacies, patients, and families. Responds to concerns voiced by patients and patient care staff in the Hospital and works closely with the patient advocate and risk management. Serves as the liaison between home health, outcome managers, documentation specialists, and the physician Hospitalist team. Completing documents for patients such as FMLA and insurance companies related to medications. Position involves frequent phone communications, extended periods of sitting, ability to walk throughout the campus executing job responsibilities, significant computer work and evaluation of clinical records, strong organizational and record keeping skills, effective communication skills needed.

Five years of experience preferred. Acute Care experience required. Current NC RN/LPN license required. Knowledge of MRH processes and medical providers required. Comfortable with using Epic.
