As a Nurse Practitioner supporting Home Health, Hospice, and Palliative Care, you will:

    Conduct patient visits across all three service linesCollaborate and participate in weekly interdisciplinary group (IDG) meetingsProvide clinical guidance and continuity of carePlay a key role in shaping patient-centered outcomes

Requirements:

    Graduate of an approved Nurse Practitioner curriculumActive APRN or NP license requiredMinimum of two years clinical nursing experienceValid driver’s license
