SCHEDULE:

This is a per diem position requiring a minimum commitment of one standing weekly shift. Available standing shift options include Sunday-Saturday overnight hours between 11p-8a (shifts may vary).

In addition to the required standing shift, team members may pick up other available shifts each week based on departmental needs and staffing availability. As a per diem employee, total scheduled hours may not exceed 19 hours per week. Flexibility to work additional open shifts is encouraged and appreciated.

SUMMARY:

Family Service League is seeking a per diem Social Worked for crisis services, which includes a 24/7 intensive crisis stabilization center, DASH- Diagnostic Assessment Stabilization Hub, in Hauppauge and mobile crisis teams which provides tele-crisis and in-community services. The Social Worker provides in person and telephonic crisis intervention, de-escalation and assessment as part of Mobile Crisis.

RESPONSIBILITIES:

  • The Social Worker will conduct clinical crisis assessments as part of a Mobile Crisis team serving all of Suffolk County for adults, adolescents and children with the full range of DSM diagnoses, including both mental health and substance use disorders.
  • Screen and collect referrals and collateral information from third party callers to inform assessment and linkage to immediate and long term supports.
  • Provide telephonic triage, assessment and de-escalation to determine the best disposition for individuals in crisis.
  • Utilize resources such as PSYCKES to review relevant clinical information.
  • Work closely with law enforcement to accept referrals and provide a co-response to individuals in crisis, when clinically necessary.
  • The Social Worker will complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
  • Conduct clinical crisis and risk assessments and stabilization services to adults, adolescents and children with the full range of DSM diagnoses (including both mental health and substance use disorders).
  • Provide in person/in community de-escalation, clinical assessment and linkage to individuals in crisis.
  • Coordinate with outside providers and collateral contacts.
  • Work as part of a multidisciplinary team, conferencing cases, communicating outcomes and providing follow up.
  • The Social Worker will attend routine supervision/team meetings.
  • Complete training and maintain proficiency with de-escalation skills.
  • Establish and maintain positive working relationships with others both internally and externally to achieve goals of the organization.
  • Foster Team work, work cooperatively and effectively with others to set goals, resolve problems and make decisions.
  • Assess situations to determine the importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
  • The social worker will set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information and activities.
  • Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.
  • This position is essential to the safe and continuous operation of the program. Essential employees are expected to report to work during inclement weather and other relevant emergency situations to ensure uninterrupted care and support for clients.
  • All other duties as assigned.

QUALIFICATIONS:

A Master’s Degree in a relevant clinical field is required.

Clinical Licensure (LMSW, LCSW, LMHC) is required.

At least six months experience in a behavioral health setting is required.

Clinical experience with mental health and/or substance use disorders is preferred.

Proficient computer skills, including Microsoft Office, are required.

Experience with an Electronic Health Record is preferred.

Excellent interpersonal and verbal and written communication skills are required.

Ability to problem solve, multitask, make clear decisions and work as part of a team are required.

Bilingual in Spanish is preferred.


PHYSICAL REQUIREMENTS:

Ability to sit and stand for long periods of time.
