About the Role:

The Case Manager plays a pivotal role in coordinating comprehensive care and support services for individuals and families within the health care and social assistance sector. This position is responsible for assessing client needs, developing personalized care plans, and facilitating access to appropriate resources to promote well-being and recovery. The Case Manager collaborates closely with multidisciplinary teams, including mental health professionals, social workers, and community organizations, to ensure seamless service delivery and continuity of care. A key outcome of this role is to empower clients through advocacy, education, and crisis intervention, ultimately improving their quality of life and social functioning. The Case Manager also monitors progress, adjusts plans as necessary, and documents all interactions to maintain compliance with regulatory standards and organizational policies.

Minimum Qualifications:

  • Associates degree
  • At least two years of experience in case management or social services within a healthcare or community setting working with women and children.
  • Knowledge of mental health issues, crisis intervention techniques, and discharge planning processes.
  • Strong understanding of child welfare systems and human services programs.
  • Excellent communication, organizational, and documentation skills.
  • Clean driving record

Preferred Qualifications:

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
  • Familiarity with conflict resolution strategies and trauma-informed care practices.
  • Proficiency with electronic health records (EHR) and case management software.

Responsibilities:

  • Coordinate and facilitate access to social services, mental health resources, and community programs to support client goals.
  • Provide crisis intervention and conflict resolution to address immediate client needs and stabilize situations.
  • Collaborate with healthcare providers, social service agencies, and child welfare organizations to ensure integrated care delivery.
  • Maintain detailed case records, prepare reports, and ensure compliance with legal and ethical standards in all case management activities.
