Oakwood Community Living Center is looking for an Admissions/Social Worker to join our growing team!

The Admissions/Social Worker is responsible for supporting residents and families through the admissions process while addressing residents’ psychosocial, emotional, and social needs throughout their stay. This position works closely with the interdisciplinary team to ensure residents receive compassionate, holistic care while maintaining dignity, independence, and quality of life.

Admissions/Social Worker duties include but are not limited to:

  • Conduct comprehensive psychosocial assessments and develop individualized care plans
  • Provide emotional support and counseling to residents and family members
  • Assist residents in accessing community resources and support services
  • Coordinate admissions and maintain the facility waiting list
  • Collaborate with nurses, physicians, therapists, psychiatrists, psychologists, and other healthcare professionals to support resident care
  • Participate in care planning meetings and advocate for residents’ psychosocial needs
  • Serve as a liaison between residents, families, community agencies, and outside providers
  • Assist with arranging adaptive equipment, transportation, dental care, vision services, hearing services, and other personal needs
  • Facilitate activities and opportunities that promote resident socialization and emotional well-being
  • Maintain accurate, timely, and confidential documentation
  • Ensure compliance with ethical standards, resident rights, and confidentiality guidelines
  • Stay current on regulations, laws, and best practices related to social work and long-term care

Requirements:
  • Bachelor’s or master’s degree in social work from an accredited program required
  • State licensure as a Social Worker preferred or required based on state regulations
  • Previous experience in a healthcare, rehabilitation, or nursing home setting preferred
