Citizens Memorial Hospital (CMH) is hiring a Care Coordinator to join our Emergency Department team in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

Hours: Mon -Fri, 10:00am-6:00pm, Flexible
What Your Day Looks Like: Care coordination advances population health by ensuring patients discharged from the ED are successfully connected to primary, specialty, and community-based services to promote the right care at the right time in the right setting. This non-clinical role plays a vital part in improving access to care, reducing avoidable emergency department utilization, and supporting positive patient outcomes through proactive care coordination.
As a Care Coordinator, you will be:

Coordinating emergency department follow-up appointments with primary care providers, specialists, and ancillary services.

Assisting patients in establishing primary care relationships when needed.

Facilitating referrals and supporting continuity of care processes

Collaborating with physicians, nurses, clinic teams, and community partners to ensure seamless care transitions

Managing scheduling, referral tracking, and electronic work queues

Supporting population health and patient experience initiatives
Qualifications:
Education: High School diploma or equivalent
Experience: Minimum one year of healthcare-related experience
Skills: Strong communication, customer service, organization, ability to manage multiple priorities in a fast-paces environment, and proficiency with electronic medical systems
Licensure/Certification: N/A
Physical Requirements: Ability to work at a computer 90% of the day and stand for the remaining 10%
