Care Coordinator - Outpatient
Job Summary: The Outpatient Care Coordinator will provide therapeutic case management and work collaboratively with outpatient psychiatric providers, the client, and external stakeholders to ensure timely coordination of outpatient aftercare following an inpatient admission. This position requires strong organization skills, the ability to work with an interdisciplinary team, strong documentation skills, and competency in the use of the electronic health record.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Core Standards & Competencies:
- Meet with clients based on discharge needs identified by the attending inpatient social workers to complete tasks that support aftercare goals and transition care to the Discharge Clinic.
- Work collaboratively with the inpatient Care Coordinators and the outpatient Practice Management team to ensure transition of care.
- Ensure that treatment plans are completed in the required timeframe, will review treatment plan with clients/guardians and request required signature(s).
- Collaborate with outside providers, referral sources, and serve as primary contact for families and guardians.
- Ensure comprehensive aftercare planning (including providing education to clients and guardians about aftercare resources, scheduling medical and mental health follow up appointments, and working with outpatient medical assistants to complete prior authorization for medications as needed).
- Demonstrate a basic knowledge of community resources related to but not limited to mental health, substance use, economic assistance, housing options, and transportation.
- Provide clear and concise documentation in the form of a Contact Note outlining contact with outpatient team members, families, and guardians during Discharge Clinic treatment.

May be required to perform duties of lesser, equal or greater responsibility.
Requirements:QUALIFICATIONS:
- Minimum of a Bachelor’s degree in human services or related field required.
- Minimum of two years of experience working in a human services field.
