What You'll Do
- Develop individualized Wellness Coordination and Risk Plans.
- Coordinate healthcare services and collaborate with physicians, providers, and interdisciplinary teams.
- Conduct in-home wellness visits and health assessments.
- Monitor health outcomes and promote preventive care.
- Train and support Direct Support Professionals on individual health and risk plans.
- Oversee medication administration competency and compliance.
- Maintain accurate documentation and ensure compliance with Indiana state guidelines.
- Participate in Individualized Support Team meetings and advocate for the health and wellness of the individuals you serve.
What You'll Bring
- Current Indiana Registered Nurse (RN) license.
- CPR/First Aid certification (required before your first active day of employment).
- Excellent communication, organization, and time management skills.
- Reliable transportation, valid driver's license, and current automobile insurance.
- Experience supporting individuals with intellectual and developmental disabilities is preferred but not required.
