We are searching for a CCS Youth Lead Care Coordinator!

Requirements:

Applicants must have a vehicle and auto insurance, a Bachelor's Degree in Human Services or related field with at least one-year of related experience, strong time management, organizational and communication skills.

Job Duties Include:

  • Provides services for approximately ten to twelve CCS members. *
  • Meets productivity benchmark of at least 20 hours a week documented in Avatar.*
  • Serves as a positive role model to Care Coordinators through appropriate and timely communication, positive attitude, and demonstrates meeting the productivity benchmark. *
  • Conduct client visits out in the community and/or in clients' homes. *
  • Assists the direct supervisors with providing feedback and coaching through a variety of different experiences, both in the office and in the community. *
  • Supports supervisors in interviewing, training, staff orientation, completing performance reviews, documenting behavioral observations, helping support any disciplinary actions or improvement plans, and providing supportive documentation. *
  • Ensures that new staff complete their initial CCS Orientation training as outlined by DHS 36 and relevant BHD/CCS Policy and Procedures, and ongoing staff complete 8 hours of training annually. *
  • Assists with ensuring all staff understand and complete required agency-specific training. *
  • Review provider documentation/paperwork and assist providers in being organized and efficient in their paperwork, scheduling, and day-to-day tasks. *
  • Assists supervisor in QA/QI tasks including the implementation of MC3 goals and NIATx projects and ensures ethical standards are maintained in their own work and in providers’ work with clients. *
  • Attends and participates in staff meetings, supervisor/lead meetings, clinical supervision sessions, in-services, and other meetings as necessary. *
  • Completes at least eight hours of continuing education including review of relevant policies annually. *
  • All responsibilities outlined in the job description for the Care Coordinator for the CCS members they are assigned to. *
  • Responsible for other duties as assigned. *
